摘要
目的:探讨银苓消肿丸与金黄膏外敷联合治疗痛风性关节炎对患者炎症反应的影响。方法:选取100例在河北省沧州中西医结合医院2022年7月—2023年7月进行治疗的湿热蕴结型痛风性关节炎患者,按照随机数字表法分为对照组和治疗组,每组50例,对照组进行常规西药治疗,治疗组在常规西药基础上加用银苓消肿丸及金黄膏外敷治疗,对比两组临床疗效、中医证候积分(关节疼痛、局部红肿、活动受限、发热畏寒)、炎性因子指标[白介素-6(IL-6)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)]、疼痛改善情况[关节疼痛视觉模拟评分(VAS)、关节疼痛发作次数]以及不良反应发生情况。结果:两组治疗效果比较,治疗组优于对照组,差异有统计学意义(P<0.05);治疗后两组中医证候相关积分均下降(P<0.05),且治疗组下降更为显著(P<0.05);治疗后两组IL-6、IL-1β、TNF-α水平均降低(P<0.05),且治疗组IL-6、IL-1β、TNF-α水平均低于对照组(P<0.05);治疗后两组关节疼痛发作次数、VAS评分、尿酸水平均降低(P<0.05),且与对照组相比,治疗组关节疼痛发作次数、VAS评分、尿酸水平均降低(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在痛风性关节炎中采用银苓消肿丸联合金黄膏外敷能够改善患者疼痛症状,疗效满意,且能够缓解炎性反应,安全性较高。
Objective:To investigate the effect of Yinling Xiaozhong Pill combined with external application of Jinhuang Ointment on inflammatory response in patients with gouty arthritis.Methods:One hundred patients with dampness-heat accumulation type gouty arthritis treated at Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from July 2022 to July 2023 were selected and randomly divided into a control group and a treatment group,50 cases in each group,using a random number table.The control group received conventional Western medicine treatment,while the treatment group received additional Yinling Xiaozhong Pill and external application of Jinhuang Ointment.Clinical efficacy,TCM syndrome scores(joint pain,local redness and swelling,limited mobility,fever and chills),inflammatory factor levels[interleukin-6(IL-6),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)],pain improvement[visual analog scale(VAS)for joint pain,frequency of joint pain attacks],and incidence of adverse reactions were compared between the two groups.Results:There was a statistically significant difference in treatment efficacy between the two groups(P<0.05).After treatment,TCM syndrome scores decreased in both groups(P<0.05),with a more significant decrease in the treatment group(P<0.05).Levels of IL-6,IL-1β,and TNF-αdecreased in both groups(P<0.05),with lower levels in the treatment group(P<0.05).The frequency of joint pain attacks,VAS scores,and uric acid levels decreased in both groups(P<0.05),with greater reductions in the treatment group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of Yinling Xiaozhong Pill and Jinhuang Ointment can improve pain symptoms,achieve satisfactory efficacy,alleviate inflammatory response,and demonstrate high safety in the treatment of gouty arthritis.
