摘要
目的:探讨头穴丛刺长留针联合经颅磁刺激(TMS)治疗脑卒中后运动性失语症的临床疗效。方法:选取160例脑卒中后运动性失语症患者为研究对象,按照随机数字表法分为2组,每组各80例。对照组采用头穴丛刺长留针疗法,观察组采用头穴丛刺长留针联合TMS疗法,疗程8周。比较2组患者西方失语症成套检查(WAB)四项及失语指数(AQ)评分,比较2组患者日常生活交流能力测验量表(CADOL-T)评分和神经功能缺损程度(NIHSS)评分情况;检测并比较2组患者血清多胺氧化酶(PAO)和心型脂肪酸结合蛋白(H-FABP)水平。结果:治疗后,2组WAB四项及AQ评分均高于治疗前,且观察组高于对照组(P<0.05);2组CADOL-T评分均高于治疗前,NIHSS评分低于治疗前,且观察组优于对照组(P<0.05);2组PAO和H-FABP水平均低于治疗前,且观察组低于对照组(P<0.05);观察组总有效率98.75%高于对照组88.75%(P<0.05)。结论:采用头穴丛刺长留针联合TMS疗法能够明显改善脑卒中后运动性失语症患者的言语功能,改善神经损伤程度。
Objective:To observe the clinical efficacy of scalp cluster acupuncture combined with transcranial magnetic stimu-lation(TMS)in the treatment of post-stroke motor aphasia.Methods:A total of 160 patients with post-stroke motor aphasia were selected and divided into two groups according to the random number table,with 80 patients in each group.The control group were given the long-time retention of scalp cluster acupuncture therapy,while the observation group were given the long-time retention of scalp cluster acupuncture combined with TMS therapy,and the treatment course was 8 weeks.Scores of the western aphasia battery(WAB)four-item and the Aphasia Quotient(AQ)of the two groups were compared.Scores of the Communication Activities of Dai-ly Living-Third Edition(CADOL-T)and the degree of neurological deficit reflected by National Institutes of Health Stroke Scale(NIHSS)of the two groups were compared.The levels of serum polyamine oxidase(PAO)and heart-type fatty acid binding protein(H-FABP)of the two groups were detected and compared.Results:After treatment,the WAB four-item and AQ scores of the two groups were higher than those before treatment,and it was higher in the observation group than in the control group(P<0.05);the CADOL-T scores of the two groups were higher than those before treatment,and higher in the observation group than the control(P<0.05);the PAO and H-FABP levels of the two groups were lower than those before treatment,and the observation group was lower the control group(P<0.05).The total effective rate of the observation group was 98.75%,higher than the 88.75%in the control group(P<0.05).Conclusion:The combination of scalp cluster acupuncture and TMS therapy can significantly improve the speech function and relieve the degree of nerve damage of patients with post-stroke motor aphasia.
