对比增强T1WI成像与对比增强磁敏感加权成像在肺癌脑转移瘤诊断中的应用价值

摘要 目的探究对比增强T1WI(CE-T1WI)成像与对比增强磁敏感加权成像(CE-SWI)在肺癌脑转移瘤诊断中的应用价值。方法选择80例原发性肺癌患者。所有患者均行CE-T1WI、CE-SWI检查,对比CE-T1WI及CE-SWI成像对不同病理类型肺癌脑转移瘤病灶的检出率,对比CE-T1WI及CE-SWI成像对不同区域肺癌脑转移瘤的检出率;对比CE-SWI对不同病理类型肺癌脑转移瘤患者中对比剂诱导相移(CIPS)的检出率。结果CE-SWI成像对腺癌脑转移瘤病灶检出率高于CE-T1WI(P<0.05),而两种成像方法对鳞癌、小细胞癌脑转移瘤病灶的检出率无统计学差异(P>0.05)。CE-T1WI序列对脑表浅区、颅底区肺癌脑转移瘤病灶的检出率均高于CE-SWI,但对脑部深区肺癌脑转移瘤病灶检出率低于CE-SWI,差异有统计学意义(P<0.05)。CE-SWI检查中鳞癌CIPS检出率低于腺癌、小细胞癌,差异有统计学意义(P<0.05);腺癌及小细胞癌患者CE-SWI检查中CIPS检出率对比,差异无统计学意义(P>0.05)。结论CE-T1WI与CE-SWI成像均可有效鉴别诊断肺癌脑转移瘤,但CE-SWI成像对肺腺癌脑转移瘤的检出率较高,且CE-SWI成像能较好地兼顾SWI及CE-T1WI的优势,可作为肺癌脑转移瘤患者MRI检查的有效补充。 Objective To explore the diagnostic value of contrast-enhanced T1WI(CE-T1WI)imaging and contrast-enhanced magnetosensitive weighted imaging(CE-SWI)in brain metastases of lung cancer.Methods 80 patients with primary lung cancer were selected and all underwent CE-T1WI and CE-SWI examinations.Compared the detection rates of brain metastases from lung cancer of different pathological types by CE-T1WI and CE-SWI imaging;Compared the detection rates of brain metastases from lung cancer in different regions by CE-T1WI and CE-SWI imaging;To compare the detection rate of contrast-induced phase shift(CIPS)in patients with brain metastases from lung cancer of different pathological types by CE-SWI.Results The detection rate of brain metastases from adenocarcinoma by CE-SWI imaging was higher than that by CE-T1WI(P<0.05),while there was no statistically significant difference in the detection rates of brain metastases from squamous cell carcinoma and small cell carcinoma by the 2 imaging methods(P>0.05).The detection rate of brain metastases from lung cancer in the superficial and base regions of the brain by the CE-T1WI sequence was higher than that by CE-SWI,but the detection rate of brain metastases from lung cancer in the deep regions of the brain was lower than that by CE-SWI,and the difference was statistically significant(P<0.05).The detection rate of CIPS in squamous cell carcinoma in CE-SWI examination was lower than that in adenocarcinoma and small cell carcinoma,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the detection rate of CIPS in CE-SWI examination between patients with adenocarcinoma and small cell carcinoma(P>0.05).Conclusion Both CE-T1WI and CE-SWI imaging can effectively differential diagnose brain metastases of lung cancer.However,CE-SWI imaging has a higher detection rate of brain metastases of lung adenocarcinoma,and CE-SWI imaging takes into account the advantages of both SWI and CE-T1WI well.It can be used as an effective supplement to MRI examination for patients with brain metastases of lung cancer.
