摘要
目的探讨甘油三酯-葡萄糖(TyG)指数和体重指数(BMI)的乘积(TyG-BMI)与高血压的相关性。方法基于2013年6月—2014年5月湖南省成人高血压患病率横断面调查数据,纳人年龄≥18岁且关键数据完整的研究对象4012例。采用二分类logistic回归分析法分析TyG-BMI与高血压的相关性,并通过受试者工作特征(ROC)曲线比较TyG指数和TyG-BMI识别高血压的能力。结果本研究纳人4012名研究对象,年龄为(54.6±12.6)岁,男性占40.98%(1644/4012)。高血压的患病率为38.33%(1538/4012)。Logistic回归分析显示TyG-BMI升高是高血压的独立危险因素(P<0.05)。在完全调整模型中,TyG-BMI每增加1/4,高血压患病风险增加1.017倍(OR=1.017,95%CI:1.014~1.019),TyG-BMI较高的四分位组(Q_(2)、Q_(3)、Q_(4))与最低四分位组(Q_(1))相比,患高血压风险也更高,OR分别为1.841、2.265和4.386。限制性立方样条图显示,TyG-BMI变化值与高血压发病风险之间呈线性的剂量反应关系(总趋势P<0.001)。在按年龄、性别、吸烟和饮酒状况分层的亚组分析中,TyG-BMI均与高血压呈正相关。此外,ROC曲线分析表明,与TyG指数相比,TyG-BMI对高血压具有更好的诊断价值。结论TyG-BMI是高血压的独立危险因素,过高的TyG-BMI或TyG-BMI的逐步增高都将增加高血压发病的风险,且与TyG指数相比,TyG-BMI在识别高血压方面具有更高的价值。
Objective To explore the correlation between triglyceride-glucose(TyG)index-body mass index(BMI)product(TyG-BMI)and hypertension.Methods Based on the cross-sectional survey data of adult hypertension prevalence in Hunan Province from June 2013 to May 2014,4012 subjects aged≥18 years with complete key data were included.Binary logistic regression analysis was used to analyze the correlation between TyG-BMI and hypertension,and receiver operating characteristic(ROC)curve was used to compare the ability of TyG index and TyG-BMI to identify hypertension.Results A total of 4 O12 subjects were included in this study,with an average age of(54.6±12.6)years,and males accounted for 40.98%(1644/4012).The prevalence of hypertension was 38.33%(1538/4012).Logistic regression analysis showed that elevated TyG-BMI was an independent risk factor for hypertension(P<0.05).In the fully adjusted model,each 1/4 increase in TyG-BMI was associated with a 1.017-fold increase in the risk of hypertension(OR=1.017,95%Cl:1.014-1.019).Compared with the lowest quartile group(Q_(1)),the higher quartile groups(Q_(2),Q_(3),Q_(4))of TyG-BMI had a higher risk of hypertension,with OR values of 1.841,2.265,and 4.386,respectively.Restricted cubic spline plot showed a linear dose-response relationship between TyG-BMI and the risk of hypertension(overall trend P<0.001).In subgroup analyses stratified by age,gender,smoking and drinking status,TyG-BMI was positively correlated with hypertension.In addition,ROC curve analysis showed that TyG-BMI had better diagnostic value for hypertension compared with TyG index.Conclusions TyG-BMI is an independent risk factor for hypertension.Excessively high TyG-BMI or gradual increase of TyG-BMI will increase the risk of hypertension,and TyG-BMI has higher value in identifying hypertension compared with TyG index.
