肥胖、瘦素与肠道微生物菌群的关系及免疫调节作用研究进展 被引量:2

摘要 目的 探讨肥胖、瘦素与肠道微生物菌群(GM)的关系及免疫调节作用。方法 总结肥胖引起的瘦素与GM变化,阐明瘦素与GM在肥胖相关免疫失调中的独立作用及相互调控机制,探讨瘦素联合GM治疗肥胖的策略。结果 肥胖状态下,瘦素水平会显著升高,且会出现瘦素抵抗,从而削弱其抑制食欲和促进能量消耗的作用;肥胖通常伴随GM的多样性下降及组成改变,进一步加剧肠黏膜屏障功能受损,导致内毒素易位,并诱发全身炎性因子水平的升高。瘦素与GM在肥胖相关炎性反应和免疫失衡中发挥着双向协同调节作用。靶向瘦素联合GM调控可作为治疗代谢型肥胖相关免疫紊乱的新策略。结论 该研究为干预肥胖及其并发症提供了新思路。 Objective To investigate the relationship among obesity,leptin,and gut microbiota(GM)and their immunomodulatory effects.Methods The changes in leptin and GM caused by obesity were summarized,the independent roles and mutual regulatory mechanisms of leptin and GM in obesity-related immune disorders were clarified,and the strategy of leptin combined with GM in the treatment of obesity was investigated.Results In obese conditions,leptin level significantly increases and leptin resistance occurs,thereby weakening its appetite-suppressing and energy-expenditure-promoting effects.Obesity is usually accompanied by a decrease in the diversity and composition of GM,which further exacerbates the damage to the intestinal mucosal barrier function,leads to endotoxin translocation and induces an increase in systemic inflammatory cytokine levels.Leptin and GM play a bidirectional synergistic regulatory role in obesity-related inflammatory responses and immune imbalances.Targeted leptin combined with GM regulation can serve as a new strategy for treating metabolic obesity-related immune disorders.Conclusion This study provides a new idea for intervening in obesity and its complications.
