以药学监护为导向的案例教学法用于重症营养支持教学效果分析

摘要 目的 探讨以药学监护为导向的案例教学(CBL)法在重症营养支持教学中的应用效果。方法 以1例外科术后肝性脑病和1例术后脓毒性休克合并肾功能不全患者的营养治疗为教学案例,通过临床药师主导的跨学科教学实践,系统展示药学专业知识在肝、肾功能不全患者营养处方制订中的应用。结合病例分析、监护方案优化及疗效评价,总结重症营养药学监护的核心教学内容与规范化培训流程。结果 教学案例中,临床药师通过药学监护提出的营养支持方案被医师采纳,对患者的治疗有效。基于真实病例的CBL法教学模式显著提升了临床药师对重症患者代谢特征分析、营养风险筛查及个体化治疗方案制订的实践能力。结论 以药学监护为导向的CBL法能有效增强临床药师在重症营养支持中的决策能力,通过理论与实践深度融合的教学模式,既能保障治疗的安全性和有效性,也为临床药师人才培养提供了可推广的教学模式。 Objective To evaluate the effectiveness of case-based learning(CBL)model guided by pharmaceutical care in critical care nutrition education.Methods With the nutritional therapy for one patient with postoperative hepatic encephalopathy and one patient with postoperative septic shock combined with renal dysfunction as teaching cases,the application of pharmaceutical expertise in the formulation of nutritional prescriptions for patients with liver and kidney dysfunction was systematically demonstrated through interdisciplinary sessions led by clinical pharmacists.Based on case analysis,optimization of monitoring plans,and efficacy evaluation,the core teaching content and standardized training process of pharmaceutical care for critical care nutrition were summarized.Results In the teaching cases,the nutrition support plans proposed by the clinical pharmacists through pharmaceutical care were adopted by the physician and effectively treated the patient.The CBL model based on real cases significantly improved the clinical pharmacists′skills in metabolic analysis,nutritional risk screening,and individualized treatment planning for critically ill patients.Conclusion The CBL model guided by pharmaceutical care can effectively enhance the clinical pharmacists′decision-making ability in critical care nutrition support.Through a teaching mode that deeply integrates theory and practice,it can not only ensure the safety and effectiveness of treatment,but also provide a replicable teaching mode for the training of clinical pharmacist talents.
