摘要
目的探讨脂溢性脱发方联合西药治疗对湿热内蕴型头皮脂溢性皮炎患者中医证候积分的影响。方法回顾性分析2022年9月—2024年10月济宁市兖州区人民医院收治的72例湿热内蕴型头皮脂溢性皮炎患者的临床资料,按治疗方式的不同将其分为对照组和观察组。对照组(n=40)采用酮康唑洗剂治疗,观察组(n=32)采用脂溢性脱发方联合酮康唑洗剂治疗,两组均持续治疗4周。对比两组的中医证候积分、临床疗效、马拉色菌有效清除率、不良反应发生情况。结果治疗后,观察组皮肤红斑、丘疹、鳞屑(脱屑)、瘙痒、头皮出油、脱发积分均低于对照组,组间差异有统计学意义(P<0.05)。观察组治疗总有效率为93.75%,马拉色菌有效清除率为90.62%,均高于对照组的75.00%、70.00%,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论脂溢性脱发方联合西药治疗湿热内蕴型头皮脂溢性皮炎患者的疗效确切,可减轻患者的临床症状,并在一定程度上提高马拉色菌有效清除率,且安全可靠。
Objective To explore the effect of seborrheic alopecia decoction combined with ketoconazole lotion treatment on Traditional Chinese Medicine(TCM)syndrome score in patients with scalp seborrheic dermatitis of dampness and heat intrinsic type.Methods The clinical data of 72 patients with scalp seborrheic dermatitis of dampness and heat intrinsic type admitted to Yanzhou District People’s Hospital of Jining from September 2022 to October 2024 were retrospectively analyzed,according to different treatment methods,they were divided into a control group and an observation group.The control group(n=40)was treated with ketoconazole lotion,and the observation group(n=32)was treated with seborrheic alopecia decoction combined with ketoconazole lotion,both groups were treated for 4 weeks.The TCM syndrome score,clinical effect,effective clearance rate of Malassezia and occurrence of adverse reactions were compared between the two groups.Results After treatment,the scores of skin erythema,papules,scales(desquamation),pruritus,scalp oil and hair loss in the observation group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).The total effective rate of the observation group was 93.75%,the effective clearance rate of Malassezia was 90.62%,which were higher than 75.00%and 70.00%of the control group,and the differences between the two groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Seborrheic alopecia decoction combined with ketocomazole lotion is effective in the treatment of patients with scalp seborrheic dermatitis with dampness and heat intrinsic type,which can reduce the clinical symptoms of patients,improve the effective clearance rate of Malassezia to a certain extent,and is safe and reliable.
