摘要
背景急性细菌性肠炎,合理补液、降温、对症抗菌药物治疗是基础干预方案,但部分患儿症状改善缓慢,且常规使用抗生素在杀灭致病菌的同时,也会无差异攻击肠道有益菌,易引起肠道菌群紊乱,降低肠道功能,影响患儿病情好转.目的探讨肠炎宁颗粒联合培菲康双歧杆菌三联活菌胶囊治疗急性细菌性肠炎患儿的效果.方法采用随机数字表法将湖州师范学院附属第一医院2023-01/2024-08诊治的92例急性细菌性肠炎患儿分为两组,每组46例.两组均给予基础治疗,于此基础上对照组采取培菲康双歧杆菌三联活菌胶囊治疗,观察组采取肠炎宁颗粒联合培菲康双歧杆菌三联活菌胶囊治疗.比较两组临床疗效、症状改善时间、肠道菌群、不良反应及胃动素(motilin,MOT)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、胃泌素(gastrin,GAS)、白介素8(interleukin 8,IL-8)、血管活性肠肽(vasoactive intestinal peptide,VIP)、和IL-6水平.结果观察组总有效率100.00%(46/46)高于对照组91.30%(42/46)(P<0.05);与对照组比较,观察组大便性状恢复正常时间、退热时间、大便次数恢复正常时间、止吐时间及住院时间均较短(P<0.05);治疗5 d后,观察组菌群正常89.13%(41/46)高于对照组71.74%(33/46)(P<0.05);与治疗5 d后对照组比较,观察组肠杆菌水平较低,乳杆菌、双歧杆菌水平较高(P<0.05);与对照组治疗5 d后比较,观察组血清VIP、TNF-α、MOT、IL-8、GAS、IL-6均更低(P<0.05);两组不良反应发生率对比无显著差异(P>0.05).结论肠炎宁颗粒与培菲康双歧杆菌三联活菌胶囊联合治疗急性细菌性肠炎患儿效果可靠且安全,可有效控制炎症介质表达,纠正菌群失衡,有助于恢复肠道功能,从而改善患儿临床症状.
BACKGROUND Acute bacterial enteritis has foundational intervention strategies such as reasonable fluid replenishment,fever reduction,and symptomatic antibacterial drug treatment.However,some children’s symptoms improve slowly.The routine use of antibiotics kills pathogenic bacteria,but it also indiscriminately attacks beneficial bowel bacteria,which can easily cause bowel bacteria disorders,reduce bowel function,and hinder the recovery of affected children.AIM To explore the clinical effects of Changyanning granules combined with Bifidobacterium triple viable capsules in the treatment of children with acute bacterial enteritis.METHODS A total of 92 children with acute bacterial enteritis treated in the First Affiliated Hospital of Huzhou Normal University from January 2023 to August 2024 were randomly divided into two groups,with 46 cases in each group.Both groups were given basic treatment.On this basis,the control group was treated with Bifidobacterium triple viable capsules,and the observation group was treated with Changyanning granules in combination with Bifidobacterium triple viable capsules.The clinical efficacy,time to symptom improvement,intestinal flora,adverse reactions,and the levels of motilin(MOT),tumor necrosis factor-α(TNF-α),gastrin(GAS),interleukin-8(IL-8),vasoactive intestinal peptide(VIP),and IL-6 were compared between the two groups.RESULTS The total effective rate in the observation group was 100.00%(46/46),which was higher than that of the control group[91.30%(42/46)](P<0.05).Compared with the control group,the observation group had shorter time to recovery of stool characteristics,time to fever reduction,time to recovery of stool frequency,antiemetic time,and hospitalization time(P<0.05).After 5 days of treatment,the normal flora in the observation group was 89.13%(41/46),which was higher than that of the control group[71.74%(33/46)](P<0.05);the level of Enterobacter in the observation group was lower,and the levels of Lactobacillus and Bifidobacterium were higher than those of the control group(P<0.05);and serum VIP,TNF-α,MOT,IL-8,GAS and IL-6 in the observation group were all lower than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).CONCLUSION The combined use of Changyanning granules and Bifidobacterium triple viable capsules is effective and safe for children with acute bacterial enteritis.It can effectively control the expression of inflammatory mediators,correct the flora imbalance,help to restore bowel function,and thereby improve the clinical symptoms of children.
