基于最佳证据营养管理联合膳食纤维对重症肺炎机械通气患者预后的影响 被引量:1

摘要 目的探讨基于最佳证据营养管理联合膳食纤维对重症肺炎机械通气(MV)患者预后的影响。方法选取2023年6月至2024年5月于南宁市第二人民医院五象医院重症医学科(ICU)行MV治疗的86例重症肺炎患者作为研究对象,以随机数表法分为试验组及对照组,每组各43例。对照组行常规护理,试验组行基于最佳证据营养管理联合膳食纤维护理。比较两组患者干预前后营养指标、免疫指标、预后指标、生活质量调查简表(SF-36)评分及喂养不耐受率。结果干预后试验组T淋巴细胞3^(+)、T淋巴细胞4^(+)、血清白蛋白、血浆血红蛋白水平及SF-36评分大于对照组,差异有统计学意义(P<0.05);试验组MV时间及住ICU时间短于对照组,差异有统计学意义(P<0.05);试验组喂养不耐受率低于对照组,差异有统计学意义(P<0.05)。结论对重症肺炎MV患者使用基于最佳证据营养管理联合膳食纤维护理能够改善患者预后、营养状态及免疫功能,降低喂养不耐受症状发生率,提升生活质量。 Objective To investigate the impact of evidence-based nutrition management combined with dietary fiber on prognosis in mechanically ventilated(MV)patients with severe pneumonia.Methods A Total of 86 patients with severe pneumonia who were treated with MV in the Department of Intensive Care Medicine(ICU)of Wuxiang Hospital of the Second People’s Hospital of Nanning from June 2023 to May 2024 were selected as the study subjects.They were randomly divided into an experimental group and a control group using a random number table,with 43 cases in each group.The control group received routine care,while the experimental group received evidence-based nutrition management combined with dietary fiber intervention.Nutritional indicators,immune markers,prognostic outcomes,the medical outcomes study 36-item short-form health survey(SF-36)scores,and feeding intolerance rates were compared between the two groups before and after the intervention.Results After the intervention,the experimental group exhibited significantly higher levels of cluster of differentiation 3^(+),cluster of differentiation 4^(+),serum albumin,plasma hemoglobin,and SF-36 scores compared to the control group,and the difference was statistically significant(P<0.05).The MV time and ICU stay in the experimental group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The feeding intolerance rate of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of evidence-based nutrition management combined with dietary fiber in MV patients with severe pneumonia can improve prognosis,enhance nutritional and immune status,reduce the incidence of feeding intolerance,and elevate quality of life.
