多元化健康宣教联合心理干预对肺癌合并肺炎患者心理健康和生活质量的影响 被引量:1

Effect of diversified health education combined with psychological intervention on psychological health and quality of life in patients with lung cancer complicated with pneumonia

摘要 目的探讨多元化健康宣教联合心理干预对肺癌合并肺炎患者心理健康和生活质量的影响。方法依据干预方法的不同将82例肺癌合并肺炎患者分为常规组(n=40)和观察组(n=42),常规组患者接受常规干预,观察组患者在常规干预的基础上接受多元化健康宣教联合心理干预。比较两组患者的心理健康状态[症状自评量表(SCL-90)、Herth希望量表(HHI)]、自我管理效能感[中文版癌症自我管理效能感量表(C-SUPPH)]、疾病应对方式[医学应对方式问卷(MCMQ)]、炎症指标[C反应蛋白(CRP)、降钙素原(PCT)]、抗生素使用时间和住院时间。结果干预后,两组患者SCL-90评分均低于本组干预前,HHI评分及C-SUPPH各维度评分均高于本组干预前,观察组患者SCL-90评分低于常规组,HHI评分及C-SUPPH各维度评分均高于常规组,差异均有统计学意义(P﹤0.05)。干预后,两组患者回避、屈服评分均低于本组干预前,面对评分均高于本组干预前,观察组患者面对评分高于常规组,回避、屈服评分均低于常规组,差异均有统计学意义(P﹤0.05)。干预后,两组患者CRP、PCT水平均低于本组干预前,差异均有统计学意义(P﹤0.05)。观察组患者抗生素使用时间、住院时间均短于常规组,差异均有统计学意义(P﹤0.05)。结论多元化健康宣教联合心理干预有利于肺癌合并肺炎患者的心理健康,可提高患者的自我管理效能感,促使患者以积极的方式应对疾病,有利于缩短治疗时间。 Objective To analyze the effect of diversified health education combined with psychological intervention on psychological health and quality of life in patients with lung cancer complicated with pneumonia.Method According to different intervention methods,82 patients with lung cancer complicated with pneumonia were divided into conventional group(n=40)and observation group(n=42).Patients in the conventional group received conventional intervention,while patients in the observation group received diversified health education combined with psychological intervention on the basis of conventional intervention.The psychological health[symptom check list-90(SCL-90),Herth hope index(HHI)],self-management efficacy[Chinese-strategies used by people to promote health(C-SUPPH)],disease coping strategies[medical coping modes questionnaire(MCMQ)],inflammatory indexes[C-reactive protein(CRP),procalcitonin(PCT)],antibiotic use time,and hospital stay were compared between the two groups.Result After intervention,the SCL-90 scores in both groups were lower than those before intervention,while the HHI scores and the scores of dimensions of C-SUPPH were higher than those before intervention,the SCL-90 score in observation group was lower than that in conventional group,while the HHI score and the scores of dimensions of C-SUPPH were higher than those in conventional group,and the differences were statistically significant(P<0.05).After intervention,the avoidance and surrender scores in both groups were lower than those before intervention,and the face scores were higher than those before intervention,the face score in observation group was higher than that in conventional group,and the avoidance and surrender scores were lower than those in conventional group,and the differences were statistical-ly significant(P<0.05).After intervention,the levels of CRP and PCT in both groups were lower than those before intervention,and the differences were statistically significant(P<0.05).The antibiotic use time and hospitalization time in observation group were shorter than those in conventional group,and the differences were statistically significant(P<0.05).Conclusion Diversified health education combined with psychological intervention is beneficial for the psychological health of patients with lung cancer complicated with pneumonia.It can improve self-management efficacy,encourage them to cope with the disease in a positive way,and shorten treatment time.
