摘要
目的探讨结构-过程-结果模型(SPO)在护理不良事件管理体系中的应用效果。方法医院依托SPO模型,从结构、过程和结果3个维度构建“预防-监控-改进”闭环式护理不良事件管理体系,并于2023年开始实施,设定2021—2022年为实施前阶段,2023—2024年为实施后阶段,比较实施前后患者不良事件发生情况、满意度以及护理人员风险感知和安全行为的变化。结果实施后,护理不良事件总发生率下降,其中跌倒/坠床发生率为0.24%、非计划性拔管发生率为0.08%,较实施前的0.31%和0.17%有明显改善(P<0.05)。同时,护理人员的风险感知能力和安全行为得分均较实施前显著提高(P<0.05)。结论应用SPO模型构建护理不良事件管理体系,围绕结构-过程-结果3个维度实施管理,能有效减少不良事件,提升护理人员的风险评估和处理能力,为医院不良事件管理提供长效机制的参考。
Objective To explore the application effect of the Structure-Process-Outcome(SPO)model in the management system of nursing adverse events.Methods The hospital constructed a closed-loop"preventionmonitoring-improvement"nursing adverse event management system based on the SPO model across three dimensions(structure,process,and outcome).Implementation began in 2023,with 2021 to 2022 designated as the pre-implementation phase and 2023 to 2024 as the post-implementation phase.Comparisons were made regarding the incidence of adverse events,patient satisfaction,and changes in nurses’risk perception and safety behaviors before and after implementation.Results After implementation,the total incidence of adverse nursing events decreased.The incidence of falls/bed falls was 0.24%and the incidence of unplanned extubation was 0.08%,which were significantly improved compared with 0.31%an d 0.17%before the implementation(P<0.05).Meanwhile,the risk perception ability and safety behavior scores of the nursing staff were significantly improved compared with those before implementation(P<0.05).Conclusion The SPO model-based nursing adverse event management system,focusing on structure,process,and outcome dimensions,effectively reduces adverse events and enhances nurses’risk assessment and management capabilities.This approach provides a reference for establishing a long-term mechanism in hospital adverse even t management.
