摘要
目的:探讨哮喘患儿血清微小RNA-185(miRNA-185)和细胞分裂周期蛋白42(CDC42)的表达变化及其对临床预后的预测价值。方法:选取哮喘患儿150例为哮喘患儿组,选取同期同年龄段体检健康儿童30例为健康对照组。采集治疗前、治疗第3个月和治疗第6个月的血清样本,检测miRNA-185和CDC42蛋白的表达情况。比较两组临床预后指标[哮喘控制测试(ACT)评分、第1秒用力呼气量占预测值的百分比(FEV1%pred)、急性发作频率、生活质量评分(PAQLQ)]及血液炎症指标。分析血清miRNA-185和CDC42蛋白与临床预后指标、血液炎症指标的相关性,以及miRNA-185和CDC42蛋白对哮喘患儿临床预后的预测价值。结果:治疗前哮喘患儿组血清miRNA-185和CDC42蛋白表达水平以及外周血嗜酸性粒细胞计数和血清总IgE水平高于健康对照组(均P<0.05)。随着治疗的进行,哮喘患儿组血清miRNA-185和CDC42蛋白表达水平以及外周血嗜酸性粒细胞计数和血清总IgE水平逐渐下降(均P<0.05)。治疗前哮喘患儿组ACT评分、FEV1%pred、PAQLQ评分低于健康对照组(均P<0.05)。随着治疗的进行,哮喘患儿组ACT评分、FEV1%pred、PAQLQ评分逐渐增加,急性发作频率逐渐降低(均P<0.05)。血清miRNA-185和CDC42蛋白与ACT评分、FEV1%pred、PAQLQ评分及呈负相关,与外周血嗜酸性粒细胞计数、急性发作频率及血清总IgE水平呈正相关(均P<0.05)。血清miRNA-185和CDC42蛋白对哮喘患儿的预后有良好的预测价值,两项联合的预测价值更高(均P<0.05)。结论:哮喘患儿血清miRNA-185和CDC42蛋白表达升高,与临床预后指标和血液炎症指标相关,对哮喘患儿的临床预后具有良好的预测价值。
Objective:To explore the expression changes of serum microRNA-185(miRNA-185)and cell division cycle 42(CDC42)protein in children with asthma and their value in predicting clinical prognosis.Methods:A total of 150 children with asthma were enrolled as the asthma group,and 30 healthy children who underwent physical examination at the same age during the same period were enrolled as the healthy control group.Serum samples were collected before treatment,3 and 6 months after treatment,and the expressions of miRNA-185 and CDC42 protein were detected.The clinical prognosis indicators Asthma Control Test(ACT)score,FEV1%pred,acute attack frequency,Pediatric Asthma Quality of Life Questionnaire(PAQLQ)score and blood inflammatory indicators were compared between the two groups.The correlation of serum miRNA-185 and CDC42 protein with clinical prognostic indicators and blood inflammatory indicators was analyzed,and the predictive value of miRNA-185 and CDC42 protein in the clinical prognosis of children with asthma was analyzed.Results:Before treatment,the asthma group had significantly higher serum miRNA-185 and CDC42 protein expression levels,peripheral blood eosinophil count and serum total IgE levels than the healthy control group(all P<0.05).With the progress of treatment,the expression levels of serum miRNA-185 and CDC42 protein,peripheral blood eosinophil count and serum total IgE levels in the asthma group gradually decreased(all P<0.05).Before treatment,the ACT score,FEV1%pred and PAQLQ score in the asthma group were lower than those in the healthy control group(all P<0.05).With the progress of treatment,the ACT score,FEV1%pred and PAQLQ score in the asthma group gradually increased,and the frequency of acute attack gradually decreased(all P<0.05).Serum miRNA-185 and CDC42 protein levels were negatively correlated with ACT score,FEV1%pred and PAQLQ score,and positively correlated with peripheral blood eosinophil count,acute attack frequency and serum total IgE level(all P<0.05).Serum miRNA-185 and CDC42 protein had good predictive value for the prognosis of children with asthma,and the combination of the two had a higher predictive value(all P<0.05).Conclusion:Children with asthma have elevated serum levels of miRNA-185 and CDC42 protein,which are associated with clinical prognostic indicators and blood inflammatory indicators.They have a good predictive value for the clinical prognosis of children with asthma.
