摘要
老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者出院后常面临体质量减轻、肌肉萎缩和营养不良等多重营养管理挑战,这些问题直接影响其健康恢复与生活质量。本综述旨在系统梳理近年来老年COPD患者院外营养管理领域的研究进展。现有研究主要关注营养评估、膳食管理、营养支持及行为干预等干预措施。结果显示,有效的营养管理能显著改善老年COPD患者的营养状况和生活质量,并有效降低并发症发生率和非计划再入院率。然而,尽管该领域已取得一定进展,老年COPD患者的院外营养管理仍面临诸多挑战。主要包括如何将营养干预更好地整合到临床实践中,以及如何确保持续的长期干预效果并进行有效评估。未来的研究应进一步探索更精细化、个性化的营养干预策略,加强跨学科团队合作,并充分利用互联网平台进行营养管理的追踪与评估,从而全面提升患者的健康状况和生活质量。
Elderly patients with chronic obstructive pulmonary disease(COPD)frequently encounter multiple nutritional challenges post-discharge,such as weight loss,muscle atrophy,and malnutrition,all of which directly impact their health recovery and quality of life.This review aims to systematically synthesize recent advances in the field of out-of-hospital nutritional management for elderly COPD patients.Existing research primarily focuses on interventions encompassing nutritional assessment,dietary management,nutritional support,and behavioral interventions.Findings indicate that effective nutritional management significantly improves elderly COPD patients'nutritional status and quality of life,concurrently lowering the incidence of complications and unplanned readmissions.Despite these advancements,out-ofhospital nutritional management for elderly COPD patients still faces considerable challenges.These primarily include the optimal integration of nutritional interventions into clinical practice,and ensuring the sustainability and effective evaluation of long-term interventions.Future research should delve deeper into more refined and personalized nutritional intervention strategies,enhance interdisciplinary team collaboration,and leverage internet platforms for tracking and evaluation of nutritional management.Ultimately,these efforts aim to comprehensively improve patients'health status and quality of life.
