摘要
目的探讨甘肃省武威地区腹膜透析(peritoneal dialysis,PD)患者自我管理能力的现状及相关因素。方法选取2024年1—9月武威市人民医院诊疗的74例PD患者的临床资料,依据居家腹膜透析患者自我管理能力调查问卷评分结果分为自我管理能力不良组(评分<105分)与自我管理能力良好组(评分≥105分)。记录2组患者的基线资料,采用单因素、多因素logistic回归分析观察自我管理能力的相关风险因素。结果74例PD患者依据居家腹膜透析患者自我管理能力调查问卷评分,自我管理能力不良组29例,评分为(94.51±8.63)分;自我管理能力良好组45例,评分为(163.38±9.27)分。单因素分析结果显示,2组患者的年龄、文化程度、经济收入、透析时间、寻求医护帮助等比较,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,年龄(OR=4.027,P=0.004,95%CI 3.450~7.526)、文化程度(OR=2.875,P=0.006,95%CI2.215~6.345)、经济收入(OR=3.330,P=0.003,95%CI2.877~6.021)、透析时间(OR=2.630,P=0.009,95%CI1.556~4.448)及寻求医护帮助(OR=3.800,P=0.012,95%CI3.114~7.801)均为影响PD患者自我管理能力的重要因素。结论年龄较大、文化程度较低、经济收入较差、透析时间较长、未寻求医护帮助是影响PD患者自我管理能力不良的主要因素,针对该类影响因素,需加强健康教育,提供个体化护理干预,增强患者自我管理意识,并鼓励患者主动寻求医护帮助,以改善其生活质量。
Objective To explore the current status of selfmanagement ability and related factors in peritoneal dialysis(PD)patients in Wuwei Area,Gansu Province.Methods A total of 74 PD patients treated in People′s Hospital of Wuwei from January 2024 to September 2024 were selected.Based on the scores from the home peritoneal dialysis patient self-management ability questionnaire,patients were divided into a poor self-management group(<105 points)and a good self-management group(≥105 points).The baseline data of the two groups were recorded,and univariate and multivariate logistic regression analyses were used to identify risk factors associated with self-management ability.Results A total of 74 PD patients,29 were in the poor self-management group with a score of(94.51±8.63)points,while 45 were in the good selfmanagement group with a score of(163.38±9.27)points.Univariate analysis revealed significant differences in age,education level,economic income,dialysis duration and seeking medical assistance between the two groups(P<0.05).The results of the multivariate logistic regression analysis showed that:age(OR=4.027,P=0.004,95%CI 3.450~7.526),education level(OR=2.875,P=0.006,95%CI 2.215~6.345),economic income(OR=3.330,P=0.003,95%CI 2.877~6.021),dialysis duration(OR=2.630,P=0.009,95%CI 1.556~4.448)and seeking medical assistance(OR=3.800,P=0.012,95%CI 3.114~7.801)were all important factors affecting the selfmanagement ability of patients with PD.Conclusion Older age,lower education level,poorer economic status,longer dialysis duration and lack of seeking medical assistance are the main factors associated with poor self-management ability in PD patients.To address these factors,it is necessary to strengthen health education,provide individualized nursing interventions,enhance patients′self-management awareness,and encourage patients to actively seek medical assistance to improve their quality of life.
