摘要
目的探讨腹膜透析(peritoneal dialysis,PD)患者容量管理行为的潜在类别及其影响因素。方法于2023年9—12月采用便利抽样法,选取广东省某三级甲等医院PD中心的386例患者作为研究对象。采用一般资料调查表、持续非卧床腹膜透析(continuous ambulatory peritoneal dialysis,CAPD)容量管理行为量表、容量相关知识调查表、容量管理知识掌握度评估表、社会支持评定量表进行问卷调查。运用潜在类别分析(latent class analysis,LCA)分析患者容量管理行为的潜在分型,并通过单因素分析和logistic回归分析探讨其影响因素。结果PD患者容量管理行为分为3种潜在类别:普遍中度知识水平-低级知识掌握度-中等管理行为(63.7%)、中度知识水平-低级知识掌握度-低等管理行为(24.6%)、高度知识水平-中级知识掌握度-高等管理行为(11.7%)。分析显示,性别、透析龄、职业、受教育程度、腹膜功能转运类型及社会支持水平是PD患者容量管理行为潜在类别的影响因素(P<0.05)。结论PD患者容量管理行为呈现明显异质性。专科护士应通过针对性的干预策略,切实提高患者容量管理水平。
Objective To explore the latent classes and influencing factors of volume management behaviors in patients with peritoneal dialysis(PD).Methods From September to December 2023,a convenience sampling method was adopted to recruit 386 PD patients from a tertiary hospital in Guangdong province.Questionnaire survey were conducted using the general information questionnaire,the continuous ambulatory peritoneal dialysis(CAPD)volume management behavior scale,a volume management knowledge questionnaire,a knowledge mastery assessment scale,and the social support rating scale.Latent class analysis(LCA)was employed to identify potential subtypes of volume management behaviors of patients,and univariate analysis and logistic regression analysis were conducted to determine influencing factors.Results Volume management behaviors of PD patients were classified into 3 potential categories:Moderate knowledge level-low knowledge mastery-moderate management behavior(63.7%),moderate knowledge level-low knowledge mastery-low management behavior(24.6%),and high knowledge level-intermediate knowledge mastery-high management behavior(11.7%).Multivariate analysis revealed that gender,dialysis vintage,occupation,education level,peritoneal transport type,and social support level were the influencing factors of latent class membership of volume management behaviors in patients with PD(P<0.05).Conclusion PD patients exhibit significant heterogeneity in volume management behaviors.Specialized nurses should implement stratified intervention strategies tailored to improve patients'volume management outcomes.
