超声脐血流参数在妊娠期高血压产妇胎儿宫内缺氧预测中的应用价值

Application value of ultrasonic umbilical blood flow parameters in predicting fetal intrauterine hypoxia in pregnant women with gestational hypertension

摘要 目的:探究超声脐血流参数在妊娠期高血压产妇胎儿宫内缺氧预测中的应用价值。方法:选取2022年9月至2024年6月南阳豫西健和医院收治的62例妊娠期高血压产妇的临床及随访资料进行研究。所有产妇均行超声检查且测量脐血流参数[脐动脉搏动指数(peripheral resistance index,PI)、脐动脉血流阻力指数(resistance index,RI)、脐动脉血流收缩期最大血流速度与舒张期末期血流速度的比值(maximum end-systolic blood flow velocity/maximum end-diastolic blood flow velocity,S/D)],基于产妇胎儿宫内是否缺氧,将其分为正常组(n=45)与缺氧组(n=17)。比较两组PI、RI、S/D水平差异。使用受试者工作特征(receiver operating characteristic,ROC)曲线分析探讨上述指标对妊娠期高血压产妇胎儿宫内缺氧事件发生的预测价值。结果:缺氧组产妇的超声脐血流参数PI、RI、S/D水平均高于正常组(P<0.05);ROC曲线进一步分析显示,产妇PI参数用于预测胎儿宫内缺氧的曲线下面积(area under the curve,AUC)、敏感度、特异度分别为0.682、70.59%、62.22%;产妇RI参数用于预测胎儿宫内缺氧的AUC、敏感度、特异度分别为0.665、64.71%、68.89%;患者S/D参数用于预测胎儿宫内缺氧的AUC、敏感度、特异度分别为0.779、76.47%、71.11%;三者联合用于预测胎儿宫内缺氧的AUC、敏感度、特异度分别为0.885、88.24%、84.44%。结论:超声脐血流参数PI、RI、S/D可有效预测妊娠期高血压产妇胎儿宫内缺氧风险,联合应用可提高预测准确性,为临床决策提供更为准确的依据。 Objective:To explore the application value of ultrasonic umbilical blood flow parameters in predicting fetal intrauterine hypoxia in pregnant women with gestational hypertension.Methods:The clinical and follow-up data of 62 pregnant women with gestational hypertension admitted to Yuxi Jianhe Hospital of Nanyang from September 2022 to June 2024 were studied.All pregnant women underwent ultrasound examinations to measure umbilical blood flow parameters,including the umbilical artery pulsatility index(PI),resistance index(RI)and the ratio of the maximum systolic blood flow velocity to the maximum end-diastolic blood flow velocity(S/D).Based on whether there was fetal intrauterine hypoxia,the pregnant women were divided into a normal group(n=45)and a hypoxia group(n=17).The differences in PI,RI and S/D levels between the two groups were compared.The predictive value of these parameters for fetal intrauterine hypoxia in pregnant women with gestational hypertension was explored using receiver operating characteristic(ROC)curve analysis.Results:The levels of ultrasonic umbilical blood flow parameters PI,RI and S/D in the hypoxia group were higher than those in the normal group(P<0.05).Further ROC curve analysis showed that the area under the curve(AUC),sensitivity and specificity of the PI parameter for predicting fetal intrauterine hypoxia were 0.682,70.59%and 62.22%,respectively;for the RI parameter,they were 0.665,64.71%and 68.89%,respectively;for the S/D parameter,they were 0.779,76.47%and 71.11%,respectively.When the three parameters were combined for prediction,the AUC,sensitivity and specificity were 0.885,88.24%and 84.44%,respectively.Conclusion:Ultrasonic umbilical blood flow parameters PI,RI and S/D can effectively predict the risk of fetal intrauterine hypoxia in pregnant women with gestational hypertension.Their combined application can improve the accuracy of prediction and provide a more accurate basis for clinical decision-making.
