摘要
目的:探究舒腹汤加减联合抗生素治疗小儿肠系膜淋巴结炎(ML)的疗效,并观察对患儿血清炎症因子的影响。方法:选取84例ML患儿为研究对象。按照随机数字表法将其分为研究组与对照组,各42例。对照组予以抗生素治疗,研究组基于对照组加用舒腹汤加减治疗,均治疗1个月。评价两组临床疗效及用药安全性,比较组间中医证候积分、肠系膜淋巴结影像检查结果,并分析血清白介素(IL)-8、IL-6与IL-17的水平变化情况。结果:观察组总有效率95.24%,高于对照组80.95%(P<0.05);观察组腹痛、腹胀、呕吐等证候消失时间均低于对照组,且在治疗1个月后各项证候积分均低于对照组(P<0.05);两组治疗1个月后肠系膜淋巴结纵横径与血清IL-8、IL-6、IL-17水平均低于治疗前,且观察组均低于对照组(P<0.05);两组不良反应发生率均低于10%,差异无统计学意义(P>0.05)。结论:基于抗生素联合舒腹汤加减治疗小儿ML,可有效缓解腹痛等病症,缩小肠系膜淋巴结,下调炎症因子水平,疗法安全且效果优于单纯西药治疗。
Objective:To explore the efficacy of modified Shufu decoction combined with antibiotics in the treatment of mesenteric lymphadenitis(ML)in children,and to observe the influence on serum inflammatory factors.Methods:84 children with ML were divided into study group and control group,with 42 cases in each group.The control group was treated with antibiotics,while the study group was added with modified Shufu decoction based on the control group.Both groups were treated for 1 months.The clinical efficacy and medication safety in the two groups were evaluated.The TCM syndromes scores and mesenteric lymph node imaging results were compared between groups,and the levels of serum interleukin(IL)-8,IL-6 and IL-17 were analyzed.Results:The total effective rate in observation group was 95.24%,which was higher than 80.95%in control group(P<0.05).The disappearance durations of syndromes such as abdominal pain,abdominal distension and vomiting in observation group were shorter than those in control group,and the scores of various syndromes after 1 months of treatment were lower than those in control group(P<0.05).After 1 months of treatment,the longitudinal and transverse diameters of mesenteric lymph nodes and levels of serum IL-8,IL-6 and IL-17 in both groups were lower compared with those before treatment,and the above indicators in observation group were lower compared to control group(P<0.05).The incidence rates of adverse reactions in both groups were less than 10%(P>0.05).Conclusion:Combination of modified Shufu decoction based on antibiotics in the treatment of ML in children can effectively relieve the abdominal pain and other syndromes,shrink the mesenteric lymph nodes,and down-regulate the levels of inflammatory factors.The therapy is safe and its effect is better than that of western medicine alone.
