摘要
目的结合临床实践经验,探讨自拟参芪归芎方加减联合新活素治疗心力衰竭急性加重的疗效及对心功能水平的影响。方法选取2021年2月~2023年10月我院收治的86例心力衰竭急性加重患者,根据信封法,随机分为对照组和观察组。对照组给予新活素治疗,观察组在此基础上采用自拟参芪归芎方进行联合治疗,比较两组临床疗效,并对治疗前后两组患者心功能、血脂水平、氧化应激反应、NT-proBNP和6MWD水平等指标进行比较,观察两组患者并发症发生情况。结果观察组患者治疗总有效率(93.02%)明显高于对照组(74.42%)(χ^(2)=4.181,P=0.041)。与对照组相比,治疗后观察组患者LVEF水平更高,LVESD和LVEDD水平更低(P<0.05)。与对照组相比,治疗后观察组患者LDL-C、TG和TC水平更低,HDL-C水平更高(P<0.05)。与对照组相比,治疗后观察组患者MDA水平更低,CTA和SOD水平更高(P<0.05)。与对照组相比,治疗后观察组患者NT-proBNP水平更低,6MWD水平更高(P<0.05)。与对照组(23.26%)相比,观察组患者并发症发生率(4.65%)更低(χ^(2)=4.745,P=0.029)。结论自拟参芪归芎方加减联合新活素治疗心力衰竭急性加重的疗效显著,可明显提高患者心功能和血脂水平,氧化应激反应明显改善,提高步行距离,减少不良反应,值得临床进一步推广应用。
Objective Combined with clinical experience,to explore the effect of Shenqi Guixiong formula plus and minus nesiritide on acute exacerbation of heart failure and its influence on cardiac function.Methods A total of 86 patients with acute exacerbation of heart failure admitted to our hospital from February 2021 to October 2023 were randomly divided into the control group and the observation group according to the envelope method.The control group was treated with nesiritide,and the observation group received combined treatment with self-designed Shenqi Guixiong formula on this basis,and the clinical efficacy of the two groups was compared.The cardiac function,blood lipid level,oxidative stress response,NT-proBNP and 6MWD levels were compared before and after treatment,and the incidence of complications was observed.Results The total effective rate of observation group(93.02%)was significantly higher than that of control group(74.42%)(χ^(2)=4.181,P=0.041).Compared with the control group,LVEF levels were higher and LVESD and LVEDD levels were lower in observation group after treatment(P<0.05).Compared with the control group,LDL-C,TG and TC levels were lower and HDL-C levels were higher in observation group after treatment(P<0.05).Compared with the control group,MDA levels in observation group were lower,CTA and SOD levels were higher after treatment(P<0.05).Compared with the control group,the level of NT-proBNP was lower and the level of 6MWD was higher in observation group after treatment(P<0.05).Compared with the control group(23.26%),the complication rate of observation group(4.65%)was lower(χ^(2)=4.745,P=0.029).Conclusion Shenqi Guixiong formula combined with nesiritide has significant curative effect in the treatment of acute exacerbation of heart failure,which can significantly improve the cardiac function and blood lipid level of patients,improve oxidative stress reaction,increase walking distance and reduce adverse reaction,and is worthy of further clinical application.
