摘要
目的观察复方丹参滴丸、酒石酸美托洛尔联合治疗慢性心力衰竭(CHF)患者的疗效,并分析其对心功能及血清学指标的影响。方法选取本院2022年5月~2024年5月101例CHF患者分为对照组50例、研究组51例。对照组予以酒石酸美托洛尔片治疗,研究组予以复方丹参滴丸联合酒石酸美托洛尔片治疗,连续治疗3个月。观察两组疗效、不良反应及治疗前后活动耐力、心衰评分、生活质量、心功能、血流动力学[左心房平均压(MLAP)、平均二尖瓣跨瓣压差(MVPG)、肺动脉平均压(MPAP)]、氧化应激[超氧化物歧化酶(SOD)、脂质过氧化物(LPO)、丙二醛(MDA)、谷胱甘肽过氧化物酶3(GPX-3)、总抗氧化能力(T-AOC)]、血管内皮功能[内皮素-1(ET-1)、一氧化氮(NO)、血管内皮生长因子(VEGF)、降钙素基因相关肽(CGRP)]等指标水平。结果研究组总有效率高于对照组(P<0.05);研究组治疗后活动耐力高于对照组,心衰评分、生活质量评分低于对照组(P<0.05);研究组治疗后左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、MLAP、MPAP低于对照组,左心室射血分数(LVEF)、每搏输出量(SV)、MVPG高于对照组(P<0.05);研究组治疗后血清SOD、GPX-3、T-AOC、NO、VEGF、CGRP水平高于对照组,LPO、MDA、ET-1水平低于对照组(P<0.05);两组不良反应比较无统计学差异(P>0.05)。结论复方丹参滴丸联合酒石酸美托洛尔片治疗CHF患者的疗效确切,可改善活动耐力、生活质量、心功能,纠正血流动力学异常,抑制氧化应激反应,改善血管内皮功能,且安全性良好。
Objective To observe the efficacy of compound Danshen dripping pills and Metoprolol tartrate in the treatment of Chronic heart failure(CHF)patients,and analyze its impact on cardiac function and serological indicator.Methods A total of 101 patients with CHF from May 2022 to May 2024 were selected and divided into a control group of 50 patients and a study group of 51 patients.The control group was treated with metoprolol tartrate tablets,while the study group was treated with compound Danshen dripping pills combined with metoprolol tartrate tablets.The continuous treatment was 3 months.The efficacy and adverse reactions of the two groups were observed,as well as the activity tolerance,heart failure score,quality of life,cardiac function,hemodynamics[Left atrial mean pressure(MLAP),Mean mitral valve transvalvular pressure gradient(MVPG),mean pulmonary arterial pressure(MPAP)],oxidative stress[superoxide dismutase(SOD),lipid peroxide(LPO),malondialdehyde(MDA),glutathione peroxidase 3(GPX-3),total antioxidant capacity(T-AOC)],vascular endothelial function[endothelin-1(ET-1),Nitricoxide(NO),vascular endothelial growth factor(VEGF),calcitonin gene-related peptide(CGRP)]before and after treatment.Results The total effective rate of the study group was higher than that of the control group(P<0.05).The activity tolerance of the study group after treatment was higher than that of the control group,while the heart failure score and quality of life score were lower than those of the control group(P<0.05).After treatment,the left ventricular end diastolic diameter(LVEDD),Vol.43,No.8,2025 left ventricular end-systolic diameter(LVESD),MLAP and MPAP in the study group were lower than those in the control group,while the left ventricular ejection fraction(LVEF),stroke volume(SV),and MVPG were higher than those in the control group(P<0.05).After treatment,the levels of serum SOD,GPX-3,T-AOC,NO,VEGF,and CGRP in the study group were higher than those in the control group,while the levels of LPO,MDA,and ET-1 were lower than those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The efficacy of compound Danshen dripping pills combined with metoprolol tartrate tablets in treating CHF patients is well established.It can improve exercise tolerance,quality of life,and cardiac function,correct hemodynamic abnormalities,inhibit oxidative stress responses,and improve vascular endothelial function,with good safety.
