摘要
目的探讨增乳汤加减联合穴位按摩对气血虚弱型产后缺乳患者乳汁分泌及雌二醇(E2)、孕酮(P)、催乳素(PRL)水平的影响。方法选取2024年1月~2025年3月于安徽省芜湖市中医医院就诊的104例气血虚弱型产后缺乳患者,按照随机数字表法分为观察组与对照组,每组52例。对照组给予穴位按摩治疗,观察组给予增乳汤加减配合穴位按摩治疗。观察两组患者治疗后临床疗效及治疗前后中医证候积分、心理状态[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、乳房饱满程度及E2、PRL、P等性激素指标,治疗前、治疗后1d、3d、7d的泌乳量及乳房胀痛情况[视觉模拟评分法(VAS)],并记录患者母乳喂养情况。结果观察组治疗总有效率为88.46%,高于对照组的73.08%(P<0.05)。治疗后,观察组中医证候积分、SDS、SAS评分及E2、P水平低于对照组(P<0.05),乳房饱满程度及PRL水平优于对照组(P<0.05)。治疗后1d两组泌乳量均较治疗前相比无明显差异(P>0.05);治疗后3d、7d,两组泌乳量均增多,且观察组泌乳量多于对照组(P<0.05)。治疗后1d、3d、7d,两组VAS评分均降低,且观察组VAS评分低于对照组(P<0.05)。观察组母乳喂养成功率为75.00%,高于对照组的53.85%(P<0.05)。结论增乳汤加减联合穴位按摩可有效提升气血虚弱型产后缺乳患者乳汁分泌量,调节性激素水平,改善中医证候、心理状态及乳房胀痛情况,提高母乳喂养成功率。
Objective To explore the effects of modified Zengru Decoction combined with acupoint massage on lactation,estradiol(E2),progesterone(P)and prolactin(PRL)in patients with postpartum hypogalactia of Qi-blood deficiency type.Methods According to random number table method,104 patients with postpartum hypogalactia of qi-blood deficiency type in Wuhu Hospital of Traditional Chinese Medicine were divided into control group(52 cases,acupoint massage)and observation group(52 cases,modified Zengru Decoction combined with acupoint massage)between January 2024 and March 2025.The clinical curative effect after treatment,scores of TCM syndromes,psychological state[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],breast plumpness and sex hormones(E2,PRL,P)before and after treatment,lactation yield and breast distending pain[visual analogue scale(VAS)]before treatment and at 1d,3d and 7d after treatment in the two groups were observed,and breastfeeding was recorded.Results The total response rate was higher in observation group than control group(88.46%vs 73.08%,P<0.05).After treatment,scores of TCM syndromes,scores of SDS and SAS,levels of E2 and P were lower(P<0.05),breast plumpness and PRL level were better in observation group than control group(P<0.05).At 1d after treatment,the difference in lactation yield was not statistically significant compared with that before treatment in either group(P>0.05).At 3d and 7d after treatment,lactation yield was gradually increased in both groups,which was more in observation group than control group(P<0.05).At 1d,3d and 7d after treatment,VAS scores were gradually decreased in both groups,which were lower in observation group than control group(P<0.05).The success rate of breastfeeding was higher in observation group than control group(75.00%vs 53.85%,P<0.05).Conclusion Modified Zengru Decoction combined with acupoint massage can effectively increase lactation yield,regulate sex hormones,improve TCM syndromes,psychological state,breast distending pain and increase success rate of breastfeeding in patients with postpartum hypogalactia of Qi-blood deficiency type.
