摘要
目的探讨1%醋酸氢化可的松乳膏治疗慢性湿疹的临床效果及对患者皮肤屏障功能的影响。方法选取吉水县人民医院皮肤科2023年5月-2024年5月收治的60例慢性湿疹患者作为研究对象,采用随机数字表法分为对照组与研究组,各30例。对照组给予氯雷他定片治疗,研究组给予1%醋酸氢化可的松乳膏治疗。比较两组临床治疗效果、湿疹症状(浸润肥厚或苔藓样变、丘疹或丘疱疹、红斑、皮损、瘙痒)评分、皮肤屏障功能(角质层水分含量、经表皮水分丢失、角质层皮质含量)、血清炎症介质水平[肿瘤坏死因子-α(TNF-α)、白介素-17(IL-17)、白介素-23(IL-23)]。结果研究组治疗总有效率(96.67%)高于对照组(73.33%)(P<0.05);研究组治疗后湿疹各症状评分均低于对照组,角质层水分含量、皮质含量高于对照组,经皮水分丢失量低于对照组,且血清炎症介质水平低于对照组(P<0.05)。结论1%醋酸氢化可的松乳膏治疗慢性湿疹的临床效果确切,能够改善患者临床症状,提高皮肤屏障功能,降低血清炎症介质水平,值得临床应用。
Objective To investigate the clinical effect of 1%hydrocortisone acetate cream in the treatment of chronic eczema and its effect on skin barrier function.Methods A total of 60 patients with chronic eczema admitted to the Department of Dermatology,Jishui County People's Hospital from May 2023 to May 2024 were selected as the research objects.They were divided into control group and study group by random number table method,with 30 patients in each group.The control group was treated with loratadine tablets,and the study group was treated with 1%hydrocortisone acetate cream.The clinical treatment effect,eczema symptoms(infiltration hypertrophy or lichenification,papules or papules,erythema,skin lesions,pruritus)score,skin barrier function(stratum corneum water content,transepidermal water loss,stratum corneum cortex content),serum inflammatory mediator levels[tumor necrosis factor-α(TNF-α),interleukin-17(IL-17),interleukin-23(IL-23)]were compared between the two groups.Results The total effective rate of treatment in the study group(96.67%)was higher than that in the control group(73.33%)(P<0.05).After treatment,the scores of eczema symptoms in the study group were lower than those in the control group,the water content and cortical content of the stratum corneum were higher than those in the control group,the amount of transcutaneous water loss was lower than that in the control group,and the level of serum inflammatory mediators was lower than that in the control group(P<0.05).Conclusion 1%hydrocortisone acetate cream is effective in the treatment of chronic eczema,which can improve the clinical symptoms of patients,improve their skin barrier function,and reduce the level of serum inflammatory mediators.It is worthy of clinical application.
