摘要
目的 探讨阿司匹林联合硫酸氢氯吡格雷对脑梗死偏瘫患者肌力、肢体功能及血清脑源性神经营养因子(BNDF)和血管内皮生长因子(VEGF)水平的影响。方法 选取2022年2月-2024年2月余干县中医院收治的68例脑梗死偏瘫患者作为研究对象,采用随机数字表法将其分为研究组和对照组,每组34例。对照组给予患者口服阿司匹林治疗,研究组在对照组治疗方案的基础上予以硫酸氢氯吡格雷治疗。比较两组临床疗效、血液流变学指标(全血低切黏度、血浆黏度、红细胞聚集指数、血细胞比容)、四肢肌力、肢体功能、血清VEGF和BDNF水平、不良反应(皮肤过敏、黑便、恶心呕吐)发生率。结果 研究组治疗总有效率高于对照组(P<0.05);研究组治疗后血液流变学指标水平均低于对照组(P<0.05);研究组治疗后四肢肌力均高于对照组(P<0.05);研究组治疗后肢体功能均优于对照组(P<0.05);研究组治疗后血清VEGF、BDNF水平均高于对照组(P<0.05);研究组不良反应发生率低于对照组(P<0.05)。结论 阿司匹林联合硫酸氢氯吡格雷治疗脑梗死偏瘫患者的临床疗效确切,能够改善血液流变学指标水平,提高患者肌力以及肢体功能,改善患者血清VEGF、BDNF水平,且安全性较高,建议临床广泛应用。
Objective To investigate the effects of aspirin combined with clopidogrel bisulfate on muscle strength,limb function and serum levels of brain-derived neurotrophic factor(BNDF)and vascular endothelial growth factor(VEGF)in patients with cerebral infarction hemiplegia.Methods From February 2022 to February 2024,68 patients with cerebral infarction hemiplegia admitted to Yugan County Hospital of Traditional Chinese Medicine were selected as the research objects.They were divided into study group and control group by random number table method,with 34 patients in each group.The control group was treated with oral aspirin,and the study group was treated with clopidogrel bisulfate on the basis of the treatment plan of the control group.The clinical efficacy,hemorheological indexes(whole blood low shear viscosity,plasma viscosity,erythrocyte aggregation index,hematocrit),limb muscle strength,limb function,serum VEGF and BDNF levels,and incidence of adverse reactions(skin allergy,melena,nausea and vomiting)were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the levels of hemorheology indexes in the study group were lower than those in the control group(P<0.05).After treatment,the limb muscle strength of the study group was higher than that of the control group(P<0.05).After treatment,the limb function of the study group was better than that of the control group(P<0.05).After treatment,the levels of serum VEGF and BDNF in the study group were higher than those in the control group(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).Conclusion Aspirin combined with clopidogrel bisulfate has a definite clinical effect in the treatment of patients with cerebral infarction hemiplegia,which can improve the level of hemorheology indexes,improve the muscle strength and limb function,improve the serum levels of VEGF and BDNF,and has high safety.It is recommended to be widely used in clinical practice.
