摘要
目的探讨经皮穴位电刺激在取卵术中的镇痛效果。方法收集2023年2月至2024年1月在贵港市人民医院生殖中心接受体外受精-胚胎移植(IVF-ET)的不孕患者,以随机数字表分为试验组(n=50,术前及术后持续经皮穴位电刺激)、安慰组(n=50,术前短暂经皮穴位电刺激)、药物试验组(n=50,术前及术后持续经皮穴位电刺激+肌注曲马多)、药物安慰组(n=50,肌注曲马多+术前短暂经皮穴位电刺激)、药物组(n=50,仅肌注曲马多)、空白组(n=50,不给予干预),采用McGill疼痛问卷(SF-MPQ)评估6组患者取卵手术时和取卵手术结束后1 h的疼痛程度,比较6组患者不良反应及胚胎发育情况。结果6组患者取卵后疼痛评分均较取卵时降低,试验组、药物试验组取卵时及取卵结束后1 h的疼痛评估指数(PRI)、视觉模拟疼痛(VAS)和现存疼痛强度(PPI)均低于其余各组(P<0.05),且药物组、药物安慰组取卵时及取卵结束1 h评分均低于安慰组、空白组(P<0.05)。此外,6组患者间不良反应发生率、获卵率、受精率及优胚率比较,差异均无统计学意义(P>0.05)。结论经皮穴位电刺激应用于取卵术中具有较好的镇痛效果,且不会显著增加不良反应及影响胚胎发育,值得临床应用及推广。
Objective To explore the analgesic effect of transcutaneous electrical acupoint stimulation during oocyte retrieval.Methods Infertility patients who underwent in vitro fertilization-embryo transfer(IVF-ET)at the Reproductive Medicine Center of Guigang People's Hospital from February 2023 to January 2024 were collected and divided into experimental group(n=50,receiving continuous transcutaneous electrical acupoint stimulation before and after surgery),placebo group(n=50,receiving brief transcutaneous electrical acupoint stimulation before surgery),drug experimental group(n=50,receiving continuous transcutaneous electrical acupoint stimulation before and after surgery+intramuscular tramadol),drug placebo group(n=50,receiving intramuscular tramadol+brief transcutaneous electrical acupoint stimulation before surgery),drug group(n=50,receiving only intramuscular tramadol),and blank group(n=50,receiving no intervention)by random number table.And then,the short-form McGill pain questionnaire(SF-MPQ)was used to evaluate pain intensity during oocyte retrieval and at 1 hour after oocyte retrieval,and adverse reactions and embryonic development were compared among the 6 groups.Results After oocyte retrieval,pain scores in all 6 groups were lower than those during oocyte retrieval.The pain rating index(PRI),visual analogue scale(VAS)scores,and present pain intensity(PPI)at both the time of oocyte retrieval and at 1 hour after oocyte retrieval were significantly lower in the experimental group and the drug experimental group than in the remaining groups(P<0.05).Furthermore,the scores at the time of oocyte retrieval and 1 hour after oocyte retrieval were lower in the drug group and the drug placebo group than in the placebo group and the blank group(P<0.05).In addition,there was no statistically significant difference in the incidence of adverse reactions,oocyte retrieval rate,fertilization rate,and high-quality embryo rate among the 6 groups(P>0.05).Conclusion Transcutaneous electrical acupoint stimulation has a good analgesic effect in oocyte retrieval,and does not significantly increase adverse reactions or affect embryo development,which is worthy of clinical application and promotion.
