摘要
目的:分析内固定术与人工全髋关节置换术(THA)治疗老年股骨颈骨折(FNF)的效果。方法:选取2020年1月—2024年6月常熟市梅李人民医院收治的60例老年FNF患者为研究对象,采用随机数字表法将其分为对照组及研究组,各30例。对照组采用内固定术治疗,研究组采用人工THA治疗。比较两组手术相关指标、术后疼痛程度、髋关节功能及并发症发生情况。结果:研究组的术中出血量多于对照组,术后首次下地负重时间、手术时间、总住院时间均长于对照组,差异均有统计学意义(P<0.05)。术后72 h、1、2周,研究组的疼痛视觉模拟评分均低于对照组,差异均有统计学意义(P<0.05)。术后3、6个月,研究组的Harris髋关节功能评分量表评分均高于对照组,差异均有统计学意义(P<0.05)。研究组的术后并发症发生率为3.33%,与对照组的13.33%比较,差异无统计学意义(P>0.05)。结论:老年FNF患者采用人工THA治疗的术后疼痛较轻,可提高术后髋关节功能,且并发症发生风险较小,具有较高的临床应用价值。
Objective:To analyze the efficacy of internal fixation and artificial total hip arthroplasty(THA)in the treatment of femoral neck fractures(FNF)in elderly patients.Methods:A total of 60 elderly patients with FNF admitted to Changshu Meili People’s Hospital from January 2020 to June 2024 were selected as the research objects and were divided into a control group and a study group by the random number table method,with 30 cases in each group.The control group was treated with internal fixation,while the study group was treated with artificial THA.The surgery-related indicators,postoperative pain degree,hip joint function and the occurrence of complications of the two groups were compared.Results:The intraoperative blood loss in the study group was more than that in the control group,the time of first weight bearing after surgeryoperation,operation time and total hospital stay were longer than those in the control group,the differences were statistically significant(P<0.05).At 72 hours,1 and 2 weeks after surgery,the scores of the Pain Visual Analogue Scale in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).At 3 and 6 months after surgery,the scores of the Harris Hip Scale in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in the study group was 3.33%,compared with 13.33%in the control group,the difference was not statistically significant(P>0.05).Conclusion:Elderly patients with FNF treated with artificial THA have relatively mild postoperative pain,can significantly improve postoperative hip joint function,and have a low risk of complications.It has high clinical application value.
