摘要
目的探讨环磷酸腺苷活化交换蛋白1(Epac1)在胃癌组织中的表达及其临床意义。方法选择2016年1月1日至2016年12月31日在西南医科大学附属医院胃肠外科确诊并接受胃癌根治手术的65例患者,收集其胃癌组织、癌旁组织和正常组织标本。采用蛋白质印迹法检测胃癌组织、癌旁组织、正常组织,以及低分化、中分化、高分化胃癌组织中的Epac1蛋白表达水平。采用免疫组织化学染色法检测胃癌组织、癌旁组织和正常组织中Epac1表达情况;根据胃癌组织中Epac1表达情况将患者分为Epac1低表达组和Epac1高表达组,分析胃癌组织中不同Epac1表达与临床病理特征的关系。术后对65例胃癌患者进行随访,绘制Kaplan-Meier生存曲线以分析Epac1表达与预后的关系。结果胃癌组织中Epac1蛋白表达水平分别显著高于癌旁组织和正常组织(P均<0.01),低分化胃癌组织中Epac1蛋白表达水平分别显著高于中分化、高分化胃癌组织(P均<0.01)。胃癌组织中Epac1表达与肿瘤浸润深度、淋巴结转移、远处转移、血管侵犯、TNM分期、分化程度均有相关性(P均<0.05),而与患者的性别、年龄、肿瘤部位、肿瘤直径和肿瘤分型均无相关性(P均>0.05)。Kaplan-Meier生存曲线分析结果显示,Epac1高表达组术后3年的总生存率和无病生存率均显著低于Epac1低表达组(P均<0.05)。结论胃癌组织中Epac1表达显著上调,这与胃癌患者预后不良相关,提示Epac1可能参与了胃癌的发生和进展,其对胃癌患者预后有一定的预测价值。
Objective This paper intends to explore the expression and clinical significance of the exchange protein directly activated by cAMP1(Epac1)in gastric cancer tissue.Methods A total of 65 patients who were diagnosed and underwent radical gastrectomy in the Department of Gastrointestinal Surgery at the Affiliated Hospital of Southwest Medical University from January 1,2016 to December 31,2016 were selected and their specimens of gastric cancer tissue,adjacent tissue,and normal tissue were collected.Western blotting was used to detect the expression levels of Epac1 protein in gastric cancer tissues,adjacent tissues,normal tissues,as well as poorly differentiated,moderately differentiated,and well differentiated gastric cancer tissues.Immunohistochemical staining was used to detect the expression of Epac1 in gastric cancer tissue,adjacent tissue,and normal tissue.Based on the expression of Epac1 in gastric cancer tissue,the patients were divided into the Epac1 low expression group and the Epac1 high expression group,and the relationship between different Epac1 expressions and clinical pathological characteristics in gastric cancer tissue was analyzed.Follow up was conducted on 65 gastric cancer patients after surgery,and Kaplan-Meier survival curves were plotted to analyze the relationship between Epac1 expression and prognosis.Results The expression levels of Epac1 protein in gastric cancer tissues are significantly higher than those in adjacent and normal tissues(P<0.01),and the expression levels of Epac1 protein in poorly differentiated gastric cancer tissues are significantly higher than those in moderately and well differentiated gastric cancer tissues(P<0.01).The expression of Epac1 in gastric cancer tissue is correlated with tumor infiltration depth,lymph node metastasis,distant metastasis,vascular invasion,TNM staging,and differentiation degree(P<0.05),but not with patient gender,age,tumor location,tumor diameter,and tumor type(P>0.05).The Kaplan-Meier survival curve analysis shows that the overall survival rate and disease-free survival rate in the Epac1 high expression group are significantly lower than those in the Epac1 low expression group 3 years after surgery(P<0.05).Conclusion The expression of Epac1 is up-regulated in gastric cancer tissues,which is associated with poor prognosis in gastric cancer patients,suggesting that Epac1 may be involved in the occurrence and progression of gastric cancer,and has certain predictive value for the prognosis of gastric cancer patients.
