从铁死亡探讨中药抑制类风湿关节炎研究进展

Exploration of the Research Progress of Traditional Chinese Medicine in Inhibiting Rheumatoid Arthritis from the Perspective of Ferroptosis

摘要 类风湿关节炎(rheumatoid arthritis,RA)是一种慢性自身免疫性疾病,以关节炎症和骨侵蚀为特征,严重影响患者生活质量。传统治疗方法存在耐受性、不良反应等问题,亟待开发新疗法。近年来,铁死亡作为一种新型细胞程序性死亡方式,因其与铁代谢紊乱和脂质过氧化的关联,为RA治疗提供了新视角。铁死亡特征为细胞内铁积累和多不饱和脂肪酸引发的脂质过氧化,与细胞氧化还原状态密切相关。在RA中,铁代谢紊乱和氧化应激增加,可能通过调节免疫反应和细胞存活影响疾病进展。该文聚焦铁死亡在RA中的作用机制,探讨其与炎症反应的关系,并分析中药通过调节ASK1/p38/MAPK、PI3K/AKT、JAK/STAT3、TNF-α/NF-κB/p38/JNK等关键信号通路对RA的抑制作用。研究表明,中药复方如逐痹汤、补肾祛寒治尪汤和青风藤水提物秦息痛等,通过靶向上述信号通路中的关键分子,有效干预RA的炎症反应和铁死亡过程,展现出治疗RA的潜力。中药成分凭借其多靶点作用的优势,为RA治疗提供了新思路。未来研究需进一步探讨中药成分对铁死亡的影响,为开发更有效、更安全的RA治疗药物提供科学依据。 Rheumatoid arthritis(RA)is a chronic autoimmune disease characterized by joint inflammation and bone erosion,which severely affects patients'quality of life.Traditional treatments face challenges such as poor tolerability and side effects,highlighting the need for new therapeutic approaches.In recent years,ferroptosis,a novel form of regulated cell death associated with iron metabolism disorders and lipid peroxidation,has emerged as a potential new target for RA treatment.Ferroptosis is characterized by the accumulation of iron within cells and lipid peroxidation triggered by polyunsaturated fatty acids(PUFAs),which is closely related to the cellular redox state.In RA,iron metabolism disorders and increased oxidative stress may influence disease progression by modulating immune responses and cell survival.This review focuses on the mechanisms of ferroptosis in RA,explores its relationship with inflammatory responses,and analyzes how traditional Chinese medicine(TCM)regulates key signaling pathways such as ASK1/p38/MAPK,PI3K/AKT,JAK/STAT3,and TNF-α/NF-κB/p38/JNK to inhibit RA.Studies have shown that TCM formulas,such as Zhubi Decoction(逐痹汤),Bushen Quhan Zhiwang Decoction(补肾祛寒治尪汤),and water extract of Qingfengteng(Sinomenii Caulis)Qinxitong(秦息痛),effectively intervene in the inflammatory response and ferroptosis process of RA by targeting key molecules in these signaling pathways,demonstrating potential for RA treatment.The multi-target action of TCM components provides a new approach for RA treatment.Future research needs to further explore the effects of TCM components on ferroptosis to provide a scientific basis for the development of more effective and safer RA treatments.
