摘要
总结施杞教授三期论治肩袖损伤的临床经验。认为肩袖损伤核心病机在内为气血失和、痰瘀痹阻,在外为筋骨失衡,主张内调气血、祛痰瘀,外调筋骨、利关节的治疗原则。根据疾病不同病理阶段的邪正盛衰及筋骨状态分为三期论治,即急性期气滞血瘀,筋伤初起,内调重在化瘀通络,外调以养筋柔筋为主;亚急性期痰瘀痹阻,骨错始现,内调重在祛痰化瘀,外调以强筋正骨为主;慢性期正气亏损而余邪未尽,筋骨粘连,内调重在扶正祛邪,外调以松解筋骨、恢复筋骨平衡为主。
This paper summarizes the clinical experience of Professor Shi Qi in the three-stage treatment of rotator cuff injury.The core pathology of rotator cuff injury is considered to involve internal disharmony of qi and blood,along with obstruction from phlegm and blood stasis,and external imbalance of the sinews and bones.The treatment principles that Professor Shi Qi advocates include internal regulation of qi and blood,elimination of phlegm and blood stasis,and external adjustment of sinews and bones to promote joint flexibility.Based on the different pathological stages of the disease,the excess or deficiency of pathogenic and healthy qi and the condition of the sinews and bones,treatment is divided into three stages:in the acute phase,characterized by qi stagnation and blood stasis,and the initial injury of the sinews,internal treatment focuses on resolving blood stasis and promoting collateral circulation,while external treatment emphasizes nourishing and softening the sinews;in the subacute phase,when phlegm and blood stasis obstruct the channels and bone displacement begins,internal treatment focuses on eliminating phlegm and resolving blood stasis,while external treatment aims at strengthening the sinews and and realigning the bones;in the chronic phase,where healthy qi is depleted and residual pathogenic qi remain,leading to adhesions of the sinews and bones,internal treatment emphasizes supporting the healthy qi and expelling pathogens,while external treatment focuses on loosening the sinews and bones and restoring the balance of the sinews and bones.
