摘要
目的:运用数据挖掘技术分析针灸治疗慢性癌症相关性疼痛(CCRP)的选穴规律。方法:通过检索2000年1月1日—2024年3月27日中国知网、维普、万方等8个国内外权威数据库针灸治疗CCRP的临床随机对照试验文献,建立(CCRP)针灸穴位处方数据库。运用统计学软件SPSS Modeler 18.0中Aprior算法深入分析处方穴位间的关联规则,借助Cytoscape 3.9.1软件绘制高频腧穴关联规则网络图,并使用SPSS Statistics 27.0软件开展高频腧穴的聚类分析,绘制聚类分析树状图。结果:经系统检索筛选,共有113篇有效文献符合纳入标准,统计出197条针灸穴位处方,涉及腧穴共116个,总频次1143次。腧穴使用频次排名前5位的依次是足三里、合谷、内关、阿是穴和三阴交;所选经脉主要是足阳明胃经、任脉和足太阳膀胱经;选穴多集中在四肢和胸腹部;特定穴中使用频次较高的是五输穴、交会穴和原穴;腧穴配伍中,相关性最高的穴组是三阴交-足三里、合谷-足三里;前12位高频腧穴可分为3个有效聚类群,C1为关元、气海、中脘、阳陵泉、肝俞、肺俞,C2为足三里、三阴交,C3为合谷、内关、太冲。结论:针灸治疗CCRP核心穴组为足三里-三阴交-合谷-内关,同时以痛为腧,重视阿是穴的选用,在此基础上增加病变脏腑的背俞穴如肝俞、肺俞等,体现了脏腑辨证、远近配穴的指导思想。
Objective:To analyze the acupoint selection rules in acupuncture treatment for chronic cancer-related pain(CCRP)using data mining techniques.Methods:A database of acupuncture acupoint prescriptions for CCRP was established by retrieving clinical randomized controlled trial literature on acupuncture treatment for CCRP from eight authoritative domestic and international databases,including CNKI,VIP,and Wanfang,from January 1,2000,to March 27,2024.The Apriori algorithm in SPSS Modeler 18.0 statistical software was employed to deeply analyze the association rules among the acupoints in the prescriptions.The Cytoscape 3.9.1 software was used to construct a network diagram of association rules for high-frequency acupoints,and SPSS Statistics 27.0 software was applied to conduct cluster analysis of highfrequency acupoints and draw a dendrogram.Results:After systematic retrieval and screening,a total of 113 valid articles met the inclusion criteria,yielding 197 acupuncture acupoint prescriptions involving 116 acupoints with a total frequency of 1,143.The top 5 acupoints in terms of usage frequency were Zusanli,Hegu,Neiguan,Ashi points,and Sanyinjiao.The selected meridians were primarily the stomach meridian of foot-yangming,the conception vessel,and the bladder meridian of foot-taiyang.Acupoint selection was mainly concentrated in the limbs and thoracoabdominal regions.Among the specific acupoints,the Five-shu acupoint,Confluent acupoint,and Source acupoint were frequently used.In terms of acupoint compatibility,the acupoint pairs with the highest correlation were Sanyinjiao-Zusanli and Hegu-Zusanli.The top 12 highfrequency acupoints could be divided into three effective clusters:C1 included Guanyuan,Qihai,Zhongwan,Yanglingquan,Ganshu,and Feishu;C2 included Zusanli and Sanyinjiao;and C3 included Hegu,Neiguan,and Taichong.Conclusion:The core acupoint group for acupuncture treatment of CCRP consists of Zusanli-Sanyinjiao-Hegu-Neiguan,with emphasis on the selection of Ashi acupoint based on the principle of“treating where the pain is.”Additionally,Beishu acupoints of the affected organs,such as Ganshu and Feishu,are added,reflecting the guiding principles of syndrome differentiation based on Zang-fu organs and the combination of distant and local acupoint selection.
