摘要
目的:通过数据挖掘技术探究穴位疗法促进腹腔镜胆囊切除术后患者胃肠功能恢复的选穴规律。方法:检索中国知网、维普、万方、中国生物医学文献服务系统、EmBase、PubMed、Web of Science、Cochrane Library数据库建库至2024年7月8日收录的关于穴位疗法促进腹腔镜胆囊切除术后胃肠功能恢复的文献。建立穴位疗法数据库,用Excel 2019、SPSS Statistics 26.0及SPSS Modeler 18.0进行数据分析。结果:最终纳入文献159篇,处方159条,腧穴49个,总频次471次,总样本量15596例,高频腧穴为足三里、内关、合谷,高频经络为足阳明胃经、手厥阴心包经、任脉,多选取下肢部腧穴,常用的穴位疗法为穴位贴敷,聚类分析得到3类有效聚类组合,关联规则分析发现足三里、内关、合谷之间存在强关联。结论:穴位疗法促进LC术后胃肠功能恢复的取穴以病变脏腑本经选穴且上下配穴为主,配合辨证取穴,以达调和脏腑、健脾和胃、降逆止呃的功效,核心穴组为足三里-内关-合谷,可为临床选穴提供参考。
Objective:Using data mining techniques to explore acupoint selection rules for gastrointestinal recovery post-laparoscopic cholecystectomy.Methods:Collated literature on acupoint therapy for the promotion of post-laparoscopic cholecystectomy gastrointestinal function recovery from CNKI,VIP,WanFang,SinoMed,EmBase,PubMed,Web of Science,and Cochrane Library in both Chinese and English databases until 8 July 2024.Subsequently,data analysis will be conducted using Excel 2019,SPSS Statistics 26.0,and SPSS Modeler 18.0 to establish a database of acupoint therapy.Results:A sample of 159 articles analyzed 159 prescriptions and 49 acupuncture points across 471 applications,totaling 15596 cases.The most frequently used acupoints were Zusanli(ST36),Neiguan(PC6),and Hegu(LI4).High-frequency meridians included the stomach(foot-Yangming),Pericardium Channel of the Hand,and Ren meridian,Cluster analysis identified 3 effective combinations.Association rules showed a strong link between Zusanli(ST36),Neiguan(PC6),and Hegu(LI4).Conclusion:Acupointtherapy enhances GI recovery after laparoscopic surgery.The acupoint selection is based on the pathology of the organ meridians and the combining of top and bottom acupoints,in conjunction with syndromic differentiation.This approach primarily promotes organ harmonization,enhances spleen and stomach function,and mitigates reflux.The core acupoint group,comprising Zusanli(ST36),Neiguan(PC6),and Hegu(LI4),provides a reference point for the selection of clinical acupoints.
