摘要
目的:分析在宫外孕破裂和卵巢黄体破裂的临床鉴别诊断中应用经阴道彩色多普勒超声(阴道彩超)的作用和价值。方法:选取2022年5月—2024年5月台山市妇幼保健院收治的宫外孕破裂和卵巢黄体破裂患者作为研究对象,分别为62例和53例。所有患者均进行阴道彩超检查,针对不同疾病患者的彩超指标、血流特征、诊断效能和病理诊断一致性进行比较和计算。结果:卵巢黄体破裂的卵巢体积、液性暗区深度以及异常回声区直径高于宫外孕破裂,差异有统计学意义(P<0.05)。两种疾病患者血流信号形态比较,差异有统计学意义(P<0.05)。宫外孕破裂的图像中主要发现了点状、条状血流信号,而卵巢黄体破裂的图像中环状血流信号更多。阴道彩超鉴别诊断两种病变效能指标中准确度95.65%、敏感度94.34%、特异度96.77%、阳性预测值96.15%、阴性预测值95.24%,且与病理诊断具有极高的一致性。结论:宫外孕破裂、卵巢黄体破裂的临床检查与鉴别诊断应用阴道彩超效果确切,效能良好,具有良好的应用效果。
Objective:To analyze the role and value of transvaginal color Doppler ultrasound(transvaginal color Doppler ultrasound)in the clinical differential diagnosis of ectopic pregnancy rupture and ovarian corpus luteum rupture.Method:Patients with ruptured ectopic pregnancy and ruptured corpus luteum of the ovary admitted to Taishan Maternal and Child Health Hospital from May 2022 to May 2024 were selected as the research subjects,with 62 cases and 53 cases respectively.All patients underwent vaginal color Doppler ultrasound examination.The color Doppler ultrasound indicators,blood flow characteristics,diagnostic efficacy and consistency of pathological diagnosis of patients with different diseases were compared and calculated.Result:The ovarian volume,depth of the fluid dark area and diameter of the abnormal echo area in ruptured corpus luteum of the ovary were higher than those in ruptured ectopic pregnancy,and the difference was statistically significant(P<0.05).The comparison of blood flow signal patterns in patients with the two diseases showed statistically significant difference(P<0.05).In the images of ruptured ectopic pregnancy,point-like and strip-like blood flow signals were mainly found,while in the images of ruptured corpus luteum of the ovary,there were more circular blood flow signals.Among the efficacy indicators of vaginal color Doppler ultrasound in the differential diagnosis of the two lesions,the accuracy was 95.65%,the sensitivity was 94.34%,the specificity was 96.77%,the positive predictive value was 96.15%,and the negative predictive value was 95.24%,and it had extremely high consistency with the pathological diagnosis(Kappa=0.912,P<0.05).Conclusion:The application of vaginal ultrasound in clinical examination and differential diagnosis of ectopic pregnancy rupture and ovarian corpus luteum rupture is accurate and effective.It has good application results.
