摘要
目的 探讨超声多模态成像技术评价滋肾育胎汤改善薄型子宫内膜容受性的应用价值。方法 按随机数字表法将本院2022年7月至2024年12月九江市中医医院接诊的76例薄型子宫内膜伴不孕患者分为观察组(n=38)、对照组(n=38)。对照组接受戊酸雌二醇片治疗,观察组加用滋肾育胎汤治疗。对比两组临床疗效、子宫内膜超声多模态评分、血清性激素指标、子宫动脉血流参数和不良反应。结果 观察组治疗总有效率94.75%,高于对照组的76.32%(P<0.05);两组治疗后子宫内膜超声多模态评分和雌激素(E_(2))、孕酮(P)水平高于治疗前,且观察组高于对照组(P<0.05);两组治疗后阻力指数(RI)、收缩期峰值流速/舒张末期流速(S/D)、搏动指数(PI)较治疗前降低,而观察组低于对照组(P<0.05);治疗过程中,两组均无明显不良反应发生。结论 滋肾育胎汤可有效改善薄型子宫内膜伴不孕的子宫动脉血流参数,改善性激素水平,安全性较高,而超声多模态成像技术可全面客观地反映薄型子宫内膜容受性。
Objective To investigate the application value of ultrasonic multimodal imaging technique in evaluating the improvement of thin endometrial receptivity by Zishen Yutai Decoction.Methods A total of 76 patients with thin endometrium and infertility treated at Jiujiang Hospital of Traditional Chinese Medicine from July 2022 to December 2024 were divided into observation group(n=38)and control group(n=38)using a random number table.The control group received estradiol valerate tablets,while the observation group was additionally treated with Zishen Yutai Decoction.The clinical efficacy,endometrial ultrasonic multimodal scores,serum sex hormone levels,uterine artery blood flow parameters,and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.75%,higher than 76.32%of the control group(P<0.05).After treatment,both groups showed increased endometrial ultrasonic multimodal scores and elevated levels of estradiol(E_(2))and progesterone(P)compared to pretreatment values,with the observation group exhibiting higher levels than the control group(P<0.05).After treatment,the resistance index(RID),systolic-to-diastolic ratio(S/D),and pulsatility index(PI)were significantly lower than pre-treatment in both groups,with the observation group demonstrating lower values than the control group(P<0.05).No significant adverse reactions were observed in either group during the treatment period.Conclusions Zishen Yutai Decoction effectively improves uterine artery blood flow parameters and sex hormone levels in patients with thin endometrium and infertility,demonstrating higer safety.Ultrasonic multimodal imaging technique provides a comprehensive and objective assessment of thin endometrial receptivity.
