摘要
目的探讨温通膏外敷联合手法松解及洛索洛芬钠凝胶膏治疗冻结肩患者的临床效果。方法选取重庆医科大学附属永川中医院2023年6月—2024年9月收治的74例冻结肩患者为研究对象,按随机数字表法将其分为对照组和观察组,每组37例。对照组采用洛索洛芬钠凝胶膏治疗,观察组在对照组基础上采用温通膏外敷联合手法松解治疗。对比两组的临床疗效、疼痛程度、肩关节功能、肩关节活动度、不良反应发生情况。结果观察组治疗总有效率为94.59%,高于对照组的78.38%,差异有统计学意义(P<0.05)。治疗后,观察组疼痛视觉模拟评分低于对照组,Constant-Murley肩关节评分量表评分高于对照组,组间差异有统计学意义(P<0.05);观察组前屈、后伸、外展、内旋、外旋活动度均大于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论温通膏外敷联合手法松解及洛索洛芬钠凝胶膏治疗冻结肩患者的效果显著,能够有效减轻疼痛,改善肩关节功能及活动度,且不良反应少。
Objective To investigate the clinical effect of external application of Wentong ointment combined with manipulation release and loxoprofen sodium gel cream in the treatment of patients with frozen shoulder.Methods A total of 74 patients with frozen shoulder admitted to Yongchuan Hospital of Traditional Chinese Medicine Affiliated to Chongqing Medical University from June 2023 to September 2024 were selected as the research objects,according to the random number table method,they were divided into a control group and an observation group,with 37 cases in each group.The control group was treated with loxoprofen sodium gel cream,and the observation group was treated with Wentong ointment combined with manual release on the basis of the control group.The clinical efficacy,pain degree,shoulder joint function,range of motion of shoulder joint,and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.59%,which was higher than 78.38%of the control group,and the difference was statistically significant(P<0.05).After treatment,the Visual Analogue Scale score of pain in the observation group was lower than that in the control group,and the Constant-Murley Shoulder scale score was higher than that in the control group,and the differences between the two groups were statistically significant(P<0.05);the range of motion of flexion,extension,abduction,internal rotation and external rotation in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion External application of Wentong ointment combined with manipulation release and loxoprofen sodium gel ointment is effective in the treatment of patients with frozen shoulder,which can effectively reduce pain,improve the function and range of motion of the shoulder joint,and has fewer adverse reactions.
