摘要
“一气周流”是黄元御学术理论的核心思想,通过对其理论深入挖掘,进一步阐释了慢阻肺的发病机制及病机演化特点,并进行分期辨证,共分为三期(初期:“土枢四象,斡旋失司,外合烟浊及六淫”为发病之始。中期:“金木失机,浊瘀积聚”为进展之关键。末期:“水火失济,阴阳相悖”为衰变之转折),并根据疾病进展及病机特点提出针对性的治疗(初期:斡旋中土,“御”气机之升降,初期重调气。中期:调畅金木,“化”浊瘀之积聚,中期重防变。末期:和合水火,“挽”阴阳于离绝,末期重衰中求生),通过对一气周流理论的研究和剖析,结合慢阻肺气道重构进展规律,构建出从整体性及动态性上把握慢性阻塞性肺病辨治思路。
"The unitary circulation of qi theory"is the core idea of Huang Yuanyu's academic theory.Through in-depth exploration of his theory,the mechanism and pathogenesis evolution characteristics of chronic obstructive pulmonary disease(COPD)are further explained,and the staged syndrome differentiation is carried out.The disease progression is divided into three stages(the initial stage:"four xiang of the earth pivot,the loss of mediation,the external combination of smoke and turbidity and six climatic exopathogens"is the beginning of the onset;the middle stage:"loss of metal and wood and the accumulation of turbidity and stasis"is the key to progress;the late stage:"water and fire are out of balance,yin and yang contradict each other"is the turning point of decay),and according to the progression of the disease and the characteristics of the pathogenesis,targeted treatment is proposed(the initial stage:mediation of the middle earth,the rise and fall of the"resisting"qi movement,focusing on readjustment of qi;the middle stage:regulating the flow of metal and wood,"dissolving"the accumulation of turbidity and stasis,focusing on preventing changes;the late stage:harmonizing water and fire,"connecting"yin and yang in separation,surviving in severe decline).Through the study and analysis of the theory of the unitary circulation of qi theory,combined with the progression patterns of COPD airway remodeling,the comprehensive and dynamic approach to treatment based on syndrome differentiation of COPD is constructed.
