摘要
目的探究腰椎减压内固定术结合神经损伤治疗仪对马尾神经损伤患者神经功能与日常生活能力的影响。方法选取2019年1月至2022年12月中国人民解放军西藏军区总医院收治的110例马尾神经损伤患者作为研究对象,按照随机数字表法分为A组与B组,各55例。A组应用腰椎减压内固定术治疗,B组应用腰椎减压内固定术联合神经损伤治疗仪治疗,比较两组临床疗效、感觉及运动功能、排尿情况、疼痛程度、下肢运动能力、日常生活能力。结果B组治疗总有效率高于A组,差异有统计学意义(P<0.05)。治疗后,B组残余尿量少于A组,视觉模拟评分法评分低于A组,最大尿流速度快于A组,自主排尿率、美国脊髓损伤协会评分、下肢运动Fugl-Meyer评分、日常生活能力指数评分均高于A组,差异有统计学意义(P<0.05)。结论腰椎减压内固定术结合神经损伤治疗仪治疗马尾神经损伤患者,可增强治疗效果,改善患者排尿能力,促进其感觉及运动功能恢复,降低其疼痛程度,改善其生活自理能力。
Objective To explore the effect of lumbar decompression and internal fixation combined with nerve injury therapeutic instrument on neurological function and daily living ability of patients with cauda equina nerve injury.Methods A total of 110 patients with cauda equina nerve injury from January 2019 to December 2022 were selected as the research subjects,and they were divided into the group A and the group B according to the random number table method,with 55 cases in each group.The group A was treated with lumbar decompression and internal fixation,while the group B was treated with lumbar decompression and internal fixation combined with nerve injury therapeutic instrument.The clinical efficacy,sensory and motor functions,urination conditions,pain degrees,lower limb motor capabilities,and activities of daily living were compared between the two groups.Results The total effective rate of treatment in the group B was higher than that in the group A,and the difference was statistically significant(P<0.05).After treatment,the residual urine volume in the group B was less than that in the group A,the visual analogue scale score was lower than that in the group A,and the maximum urine flow velocity was faster than that in the group A,the voluntary urination rate,American Spinal Injury Association score,lower extremity motor Fugl-Meyer score,and activities of daily living index score were higher than those in the group A,the differences were statistically significant(P<0.05).Conclusion Lumbar decompression and internal fixation combined with nerve injury therapeutic instrument for patients with cauda equina nerve injury can enhance the therapeutic effect,improve the patient's urination ability,promote the recovery of their sensory and motor functions,reduce their pain degree,and improve their self-care ability in daily life.
