摘要
目的:探讨当归补血汤对膝骨性关节炎(knee osteoarthritis,KOA)置换术后隐性失血的疗效。方法:选择100例行单侧膝关节置换术(total knee replacement,TKA)的KOA患者,根据随机数字表法分成两组。两组均行TKA治疗;对照组术后采取常规西医措施;观察组术后1天予当归补血汤内服。两组均连续治疗14天。比较两组隐性失血、血常规指标、美国特种外科医院膝关节评分(hospital for special surgery knee score,HSS)及并发症发生率。结果:与对照组相比,观察组隐性失血量、显性失血量、总失血量均降低(P<0.01);术后7天,两组血红蛋白(hemoglobin,Hb)、红细胞比容(hematocrit,Hct)减少,血小板计数(platelet,PLT)增加(P<0.01),且观察组Hb、PLT、Hct改善优于对照组(P<0.01);术后14天、3个月,观察组HSS评分均高于对照组(P<0.01);对照组并发症发生率[36.0%(18/50)]高于观察组[18.0%(9/50)](P<0.05)。结论:当归补血汤可减少KOA置换术后隐性失血,改善血常规指标和膝关节功能,降低并发症发生率。
Objective:To explore clinical effects of Danggui blood-enriching decoction on hidden blood loss after total knee replacement(TKA).Methods:All 100 KOA patients who performed unilateral TKA were chosen and allocated to two groups according to random number table method.Both groups were treated with TKA,and the control group adopted routine therapy of Western medicine after the operation,and the treatment group took Danggui blood-enriching decoction in one day after the surgery,for 14 days in a row.To compare occult blood loss,blood routine indexes,HSS and the incidence of the complications between the two groups.Results:Compared with the control group,occult blood loss,dominant blood loss and total blood loss in the treatment group were lowered(P<0.01);seven days after the operation,Hb and Hct were reduced,while PLT was increased in the two groups(P<0.01),and the treatment group was superior to the control group in the improvements of Hb,PLT and Hct(P<0.01);14 days and three months after the operation,the treatment group was higher than the control group in HSS scores(P<0.01);the incidence of the complications of the control group was[36.0%(18/50)],higher than[18.0%(9/50)]of the treatment group(P<0.05).Conclusion:Danggui blood-enriching decoction could alleviate hidden blood loss after TKA,improve blood routine indexes and knee function and reduce the incidence of the complications.
