摘要
目的:分析血清载脂蛋白A1(ApoA1)、尿酸(UA)、溶血磷脂酸(LPA)水平与急性缺血性脑卒中患者病情程度的相关性。方法:回顾性选取某院2021年1月—2024年3月收治的102例急性缺血性脑卒中患者为卒中组,同期102例健康体检者为健康组,比较2组入院时一般资料及血清ApoA1、UA、LPA水平。根据美国国立卫生院卒中量表(NIHSS)评分将卒中组患者分为轻度缺损组(NIHSS评分<4分,n=44)、中度缺损组(NIHSS评分4~15分,n=36)、重度缺损组(NIHSS评分>15分,n=22),比较不同病情程度患者入院时血清ApoA1、UA、LPA水平,Spearman分析血清ApoA1、UA、LPA水平与病情程度的相关性,ROC分析血清ApoA1、UA、LPA水平联合检测对急性缺血性脑卒中患者重度缺损的诊断价值。结果:卒中组血清UA、LPA水平高于健康组,血清ApoA1水平低于健康组,差异有统计学意义(P<0.05)。入院时,不同病情程度患者UA、LPA水平比较:轻度缺损组<中度缺损组<重度缺损组;血清ApoA1水平比较:轻度缺损组>中度缺损组>重度缺损组,差异均有统计学意义(P<0.05)。Spearman分析显示,血清UA、LPA水平与NIHSS评分呈正相关(r=0.654、0.674,P<0.05),血清ApoA1水平与NIHSS评分呈负相关(r=-0.624,P<0.05)。ROC分析显示,血清ApoA1、UA、LPA水平诊断急性缺血性脑卒中患者重度缺损的AUC分别为0.797、0.774、0.817;三者联合诊断的AUC为0.916。结论:血清ApoA1、UA、LPA水平与急性缺血性脑卒中患者病情进展有关,检测三者水平有助于评估病情程度,辅助临床诊断。
Objective:To analyze the correlation of of serum levels of apolipoprotein A1(ApoA1),uric acid(UA),lysophosphatidic acid(LPA)with the severity of acute ischemic stroke and its clinical significance.Methods:Patients with acute ischemic stroke in a hospital from January 2021 to March 2024 were retrospectively selected as a stroke group(n=102),and the same number of healthy people were selected as a healthy group(n=102).The National Institutes of Health Stroke Scale(NIHSS)was used to assess the severity of the disease.The clinical general data and serum levels of ApoA1,UA and LPA were compared between the two groups at admission.The levels of serum indicators in patients with different disease degrees were compared.The correlation between the levels of serum indicators and NIHSS score and the diagnostic value for patients with severe defect were analyzed.Results:The levels of serum ApoA1,UA,and LPa in the stroke group were all higher than those in the healthy group(P<0.05).The levels of UA and LPa in patients with severe defects were significantly higher than those in patients with mild and moderate defects,and the levels of UA and LPa in patients with moderate defects were higher than those in patients with mild defects.The level of serum ApoA1 in patients with mild defects was significantly higher than that in patients with moderate and severe defects,and the level of serum ApoA1 in patients with moderate defects was significantly higher than that in patients with severe defects(P<0.05).Serum UA and LPa were positively correlated with NIHSS scores(r=0.654,0.674,P<0.05),while serum ApoA1 was negatively correlated with NIHSS scores(r=-0.624,P<0.05).The AUC of serum ApoA1,UA,LPa levels alone and in combination for diagnosing severe defects in patients at admission was 0.797,0.774,0.817,and 0.916,respectively.Conclusion:The levels of serum ApoA1,UA,and LPa are involved in the progression of acute ischemic stroke patients.Detecting the levels of these three factors can help assess the severity of the disease and assist clinical diagnosis.
