摘要
目的 考察老年急性缺血性脑梗死(AIS)患者诱发老年衰弱综合征的风险因素及院内外护理对策。方法 选取2023年1月至2024年6月在徐州医科大学附属医院诊治的138例老年AIS患者进行研究,采集患者的一般资料,采用FRAIL衰弱评估量表评估患者老年衰弱综合征的发生情况,先进行单因素分析老年AIS患者诱发老年衰弱综合征的影响因素,对于存在统计学意义的临床资料,再应用Logistic回归分析风险因素,并提出院内外护理策略。结果 138例老年AIS患者中,有42例(30.43%)发生老年衰弱综合征。单因素分析结果显示,年龄、糖尿病、睡眠障碍、入院时美国国立卫生院卒中量表(NIHSS)评分、入院时白蛋白水平及脑梗死面积均是老年AIS患者诱发老年衰弱综合征的风险因素(P均<0.05)。多因素Logistic回归分析显示,年龄>75岁、患有糖尿病、合并睡眠障碍、入院时NIHSS评分≥15分、入院时白蛋白水平<35 g/L及大面积脑梗死均是老年AIS患者诱发老年衰弱综合征的危险因素(P均<0.05)。结论 年龄过大、合并睡眠障碍、患有糖尿病、营养状况不佳及神经功能缺损程度严重均是老年AIS患者发生老年衰弱综合征的危险因素,临床应积极采取院内外护理措施予以干预,以降低衰弱综合征的发生率。
Objective To investigate the risk factors of senile frailty syndrome in elderly patients with acute ischemic stroke(AIS)and in-hospital and out-of-hospital nursing countermeasures.Methods A total of 138 elderly patients with AIS treated in the Affiliated Hospital of Xuzhou Medical University from January 2023 to June 2024 were selected for the study.The general data of the patients were collected,and the FRAIL frailty assessment scale was used to evaluate the occurrence of senile frailty syndrome.Univariate analysis was performed to analyze the influencing factors of senile frailty syndrome induced by elderly AIS patients.For the clinical data with statistical significance,Logistic regression was applied to analyze the risk factors,and in-hospital and out-of-hospital nursing strategies were put forward.Results Among 138 elderly AIS patients,42(30.43%)developed senile frailty syndrome.The results of univariate analysis showed that age,diabetes mellitus,sleep disorder,National Institutes of Health Stroke Scale(NIHSS)score at admission,albumin level at admission and cerebral infarction area were the risk factors of senile frailty syndrome in elderly AIS patients(P<0.05).Multivariate Logistic regression analysis showed that age>75 years old,diabetes mellitus,sleep disorder,NIHSS score≥15 points at admission,albumin level<35 g/L at admission,and large area of cerebral infarction were the risk factors of senile frailty syndrome in elderly AIS patients(P<0.05).Conclusion Older age,sleep disorder,diabetes mellitus,poor nutritional status and severe neurological impairment are the risk factors of senile frailty syndrome in elderly AIS patients.Clinical nursing measures inside and outside the hospital should be actively taken to intervene in order to reduce the incidence of frailty syndrome.
