超脉冲CO_(2)点阵激光联合光子嫩肤治疗凹陷性痤疮瘢痕疗效及对皮肤屏障、瘢痕面积和ECCA评分的影响 被引量:1

摘要 目的观察超脉冲CO_(2)点阵激光联合光子嫩肤治疗凹陷性痤疮瘢痕疗效及对皮肤屏障指标、瘢痕面积和痤疮瘢痕临床评分量表(ECCA)评分的影响。方法选取2022年9月至2024年.9月烟台市烟台山医院就诊的凹陷性痤疮瘢痕患者148例,采用随机数表法分为联合组(74例)和对照组(74例),对照组给予超脉冲CO_(2)点阵激光治疗,联合组在超脉冲CO_(2)点阵激光治疗2周后加用光子嫩肤治疗,两组均治疗3次,每次间隔4周,比较两组末次治疗后3个月临床疗效,以及治疗前后皮肤屏障功能、瘢痕面积、ECCA评分、皮肤色度及不良反应。结果治疗后3个月,联合组总有效率高于对照组(P<0.05);两组经皮水分丢失率(TEWL)、ECCA评分及皮肤色度a*值均较治疗前显著降低(P<0.05),且联合组低于对照组(P<0.05),角质层含水量(SCH)及皮肤色度L*值均较治疗前升高(P<0.05),且联合组高于对照组;两组瘢痕面积均较显著减小(P<0.05),且联合组小于对照组(P<0.05);联合组色素沉着发生率低于对照组(P<0.05)。结论超脉冲CO_(2)点阵激光联合光子嫩肤治疗凹陷性痤疮瘢痕疗效确切,可以更有效改善皮肤屏障功能,减小瘢痕面积,降低ECCA评分,并改善皮肤色度,减少色素沉着风险,安全性良好。 Objective To observe the efficacy of ultra-pulsed CO_(2) fractional laser combined with photorejuvenation in the treatment of atrophic acne scars and its effects on skin barrier indicators,scar area,and Echelle d'évaluation clinique des cicatrices d'acné(ECCA)score.Methods A total of 148 patients with atrophic acne scars treated at Yantai Mountain Hospital were selected from September 2022 to September 2024.They were divided into a combined group(74 cases)and a control group(74 cases)using a random number table.The control group received ultra-pulsed CO_(2) fractional laser treatment,while the combined group received photorejuvenation 2 weeks after the ultra-pulsed CO_(2) fractional laser treatment.Both groups underwent 3 treatment sessions at 4-week intervals.Clinical efficacy at 3 months after the final treatment,skin barrier function,scar area,ECCA score,skin chroma,and adverse reaction Incidence were compared between the two groups before and after treatment.Results At 3 months post-treatment,the total effective rate was higher in the combined group than in the control group(P<0.05).Transepidermal water loss(TEWL),ECCA scores,and skin chroma a*values significantly decreased in both groups compared to pretreatment levels(P<0.05),with the combined group showing lower values than the control group(P<0.05).Stratum corneum hydration(SCH)and skin chroma L*values increased compared to pretreatment levels(P<0.05),with higher values in the combined group(P<0.05).Scar area significantly decreased in both groups(P<0.05),with a smaller area in the combined group(P<0.05).The incidence of pigmentation was lower in the combined group than in the control group(P<0.05).Conclusion Ultra-pulsed CO_(2) fractional laser combined with photorejuvenation is effective for treating atrophic acne scars.It better improves skin barrier function,reduces scar area,lowers ECCA scores,improves skin chroma,and decreases the risk of pigmentation,demonstrating good safety.
