摘要
椎旁恶性外周神经鞘瘤是一种非常罕见的肿瘤,其发病机制复杂。本文报道此例患者椎旁恶性外周神经鞘瘤术后伴顽固性腰腿痛,属中医“痛痹”范畴。仝小林从脏腑风湿之顶焦髓系体痹辨识本病,明确疾病核心病机为寒,外感寒湿内伏,顽寒痼冷久伤机体,方用乌头九分散,以温经散寒、祛瘀止痛、透邪外出,结合态靶辨治理论、体脏同治思想,顾护脾胃、肾关,综合辨治,获效甚佳。
Paraspinal malignant peripheral nerve sheath tumor(MPNST)is an extremely rare tumor with a complex pathogenesis.This article reports a patient whose illness was triggered by exposure to cold water,falling under the category of"painful bi"in traditional Chinese medicine(TCM).Professor Tong Xiaolin identifies the disease from the perspective of bi in the marrow system of ding jiao in the theory of zang-fu wind-damp,clarifying that its core pathogenesis lies in cold pathogen,in which external cold-dampness invades the body and stays in it,and persistent and chronic cold delivers a long-term damage to the body.Professor Tong Xiaolin applies Wutou Jiufen Powder for its treatment,warming channels for dispelling cold,removing blood stasis and relieving pain,expelling the cold pathogen.At the same time,he integrates the theory of administering treatment based on syndrome differentiation of states and targets and the idea of treating both the body and zang-fu viscera,protecting the spleen,stomach,and kidney functions,achieving significant therapeutic effects.
