摘要
目的探讨肝内胆管癌(intrahepatic cholangiocarcinoma,ICC)的临床病理学特征、免疫表型、分子生物学改变、鉴别诊断及预后。方法收集17例ICC患者手术标本的临床病理资料,采用HE和免疫组化EnVision法染色,分析其临床病理学特征,并复习相关文献。结果17例ICC患者,男11例,女6例,年龄44~81岁,65岁以上患者13列(76%),肿瘤最大径2.0~12.0 cm,腺癌15例,腺鳞癌2例,小胆管型12例,排列成密集规则的管状、条索状结构,细胞小立方形或低柱状,胞浆多嗜酸性,表达N-cadherin、CRP、CD56;大胆管型5例,排列成不规则的腺管状,细胞柱状,管腔内含黏液,表达S-100P、MUC5AC、MUC6,随访3~36个月,6例死亡。结论ICC具有不同的形态学、免疫表型、分子遗传学及预后,明确组织学分型对临床精准治疗及判断预后提供依据。
Objective To investigate the clinicopathological features and immunohistochemistry,diagnosis and differential diagnosis of intrahepatic cholangiocarcinoma(ICC).Methods Clinicopathological datas of 17 surgical specimens of ICC were collected.HE and immunohistochemical EnVision staining were used to analyze their clinicaopathological characteristics,and relevant literatures were reviewed.Results Among the 17 cases of ICC,11 were males and 6 were females,age from 44 to 81 years old,patients over 65 years old was 13 cases(accounted for 76%).The maximum diameter of the tumor was 2.0-12.0 cm,there were 15 cases of adenocarcinoma and 2 cases of adenosquamous carcinoma,and 12 cases of small bile duct type,arranged in dense and regular tubular and linear structures,the cells were small cubic or low columnar in shape,and the cytoplasm was mostly eosinophilic,expressing N-cadherin,CRP and CD56.There were 5 cases of large bile duct type,arranged in irregular glandular ducts with columnar cells and mucus in the lumen,expressing S-100P,MUC5AC and MUC6.Follow-up clinic was 3-36 months,and 6 cases died.Conclusion ICC has different morphologies,immunophenotypes,molecular genetics and prognosis.Clarifying the histological classification provides a basis for precise clinical treatment and prognosis assessment.
