摘要
目的探讨血清纤维蛋白降解复合物(DR-70)、分泌型糖蛋白(Dickkopf-1,DKK-1)、内皮细胞特异性分子(endothelial cell specific molecule-1,ESM-1)联合对结直肠癌的诊断价值。方法选取2020年2月至2023年11月确诊的112例结直肠癌患者作为研究组,选择98例良性结直肠病变患者作为良性对照组,选择98名健康体检者作为健康对照组。采用ELISA法对所有受检者血清DR-70、DKK-1、ESM-1水平进行检测;分析血清DR-70、DKK-1、ESM-1水平与结直肠癌患者临床病理的关系,采用ROC曲线分析血清DR-70、DKK-1、ESM-1水平对结直肠癌的诊断价值,比较结直肠癌患者术前术后血清DR-70、DKK-1、ESM-1水平。结果研究组患者血清DR-70、DKK-1、ESM-1水平高于良性对照组及健康对照组(P<0.05),血清DR-70、DKK-1、ESM-1水平与结直肠癌患者分化程度、临床分期、淋巴结转移及浸润深度密切相关(P<0.05);血清DR-70、DKK-1、ESM-1联合诊断结直肠癌的AUC显著大于DR-70单独诊断的AUC(Z=4.856,P<0.001),DKK-1单独诊断的AUC(Z=4.246,P<0.001),ESM-1单独诊断的AUC(Z=2.954,P=0.003)。术后血清DR-70、DKK-1、ESM-1水平均显著下降(P<0.05)。结论结直肠癌患者血清DR-70、DKK-1、ESM-1水平均升高,与患者的临床病理相关,联合检测对结直肠癌的诊断价值较好。
Objective To investigate the diagnostic value of combination of serum fibrin degradation complex(DR-70),Dickkopf-1(DKK-1)and endothelial cell specific molecule-1(ESM-1)for colorectal cancer.Methods From Feb.2020 to Nov.2023,112 patients diagnosed with colorectal cancer were regarded as the trail group,98 patients with benign colorectal lesions were selected as the benign control group,and 98 healthy examinees were as the healthy control group.ELISA was applied to detect serum levels of DR-70,DKK-1 and ESM-1 in all subjects.The relationship between serum DR-70,DKK-1,ESM-1 levels and clinical pathology of colorectal cancer patients was analyzed,ROC curve was applied to analyze the diagnostic value of serum DR-70,DKK-1 and ESM-1 levels for colorectal cancer,the serum levels of DR-70,DKK-1 and ESM-1 were compared before and after surgery in patients with colorectal cancer.Results The serum levels of DR-70,DKK-1 and ESM-1 in the trail group were higher than those in the benign control group and healthy control group(P<0.05).The levels of serum DR-70,DKK-1 and ESM-1 were closely related to the degree of differentiation,clinical stage,lymph node metastasis and depth of infiltration in colorectal cancer patients(P<0.05).The combined diagnosis of serum DR-70,DKK-1 and ESM-1 showed greatly higher AUC for colorectal cancer compared to DR-70 alone(Z=4.856,P<0.001),DKK-1 alone(Z=4.246,P<0.001)and ESM-1 alone(Z=2.954,P=0.003).The serum levels of DR-70,DKK-1 and ESM-1 greatly decreased after surgery(P<0.05).Conclusion The serum levels of DR-70,DKK-1 and ESM-1 are all elevated in patients with colorectal cancer,which is related to their clinical pathological characteristics.Combined detection has good diagnostic value for colorectal cancer.
