摘要
目的 探讨宏基因组测序进行Q热病原学诊断及其临床应用价值。方法 收集并整理病例的临床资料,开展流行病学调查,采血通过病原微生物宏基因组测序(metagenomics next-generation sequencing, mNGS)诊断。结果 病例分析显示,病例发病前曾在老家接触家养犬,且有蜱虫叮咬史。回深圳后,出现不明原因反复发热的症状,入院接受检查及治疗,mNGS诊断为贝纳柯克斯体Coxiella burnetii感染,并结合CT、彩超及血液检查,显示病例存在肝损伤。结论 通过流行病学史、临床症状及实验室检测结果,该病例被诊断为Q热合并肝损伤,这提示了mNGS在早期识别和诊断罕见感染中的重要作用。
Objective To investigate the etiological diagnosis of a case of Q fever by metagenomics next-generation sequencing(mNGS)and its clinical value.Methods Clinical data of the case were collected and organized,and an epidemiological investigation was conducted.Blood samples were collected for diagnosis by mNGS.Results Case analysis revealed that the patient had a history of contact with domestic dogs and tick bites at their hometown before the onset of illness.After returning to Shenzhen,the patient presented with recurrent fever of unknown origin.Upon admission for examination and treatment,mNGS confirmed the infection with Coxiella burnetii.Furthermore,CT scans,color Doppler ultrasound,and blood tests indicated the presence of liver injury in the patient.Conclusion Based on the epidemiological history,clinical symptoms,and laboratory test results,this case was diagnosed as Q fever complicated with liver injury.It highlights the important role of mNGS in the early identification and diagnosis of rare infections.
