PCR探针溶解曲线法在结核分枝杆菌复合群及利福平耐药性检测中的应用效果研究

Study on the Application Effect of PCR Probe Dissolution Curve Method in the Detection of Mycobacterium Tuberculosis Complex and Rifampicin Resistance

摘要 探讨PCR探针溶解曲线法对于结核分枝杆菌复合群(mycobacterium tuberculosis complex,MTBC)和利福平耐药性检测的应用效果。纳入2024年1月—2024年12月六盘水市第三人民医院接收的100例疑似肺结核患者为研究对象,采集其痰液标本,分别进行Xpert^(®)MTB/RIF(简称Xpert)、PCR荧光探针溶解曲线法检测,以结核分枝杆菌培养结果作为金标准,对比两种检测方法的效能差异。对57株MTBC菌株施以药敏试验,对比两种方法检测利福平耐药性的效果及整体成本。结果显示,100例患者中检出45例MTBC菌株阳性,阳性率为45.00%;57株MTBC菌株中检出27株存在利福平耐药性,耐药率为47.37%。PCR探针溶解曲线法与Xpert检测MTBC和利福平耐药性的灵敏度、特异度、准确率比较,无统计学差异(P>0.05)。PCR探针溶解曲线法的检测成本低于Xpert,差异有统计学意义(P<0.05)。研究发现,PCR荧光探针溶解曲线法检测MTBC及利福平耐药性的效能与与Xpert相近,但PCR荧光探针溶解曲线法具有更低的检测成本,能够避免资源浪费。 To explore the application effect of PCR probe melting curve method in the detection of Mycobacterium tuberculosis complex(MTBC)and rifampicin resistance.100 suspected tuberculosis patients admitted to the Third People’s Hospital of Liupanshui City from January 2024 to December 2024 were included as the research subjects,and their sputum samples were collected for Xpert analysis^(®)MTB/RIF(abbreviated as Xpert)and PCR fluorescence probe dissolution curve method were used for detection,and the culture results of Mycobacterium tuberculosis were used as the gold standard to compare the efficacy differences between the two detection methods.Conduct drug susceptibility testing on 57 MTBC strains and compare the effectiveness and overall cost of two methods for detecting rifampicin resistance.The results showed that 45 out of 100 patients were positive for MTBC strain,with a positivity rate of 45.00%;Out of 57 MTBC strains,27 were found to be resistant to rifampicin,with a resistance rate of 47.37%.There was no statistically significant difference(P>0.05)in the sensitivity,specificity,and accuracy of PCR probe dissolution curve method and Xpert for detecting MTBC and rifampicin resistance.The detection cost of PCR probe dissolution curve method is lower than Xpert,and the difference is statistically significant(P<0.05).Research has found that the efficacy of PCR fluorescence probe dissolution curve method in detecting MTBC and rifampicin resistance is similar to Xpert,but PCR fluorescence probe dissolution curve method has lower detection costs and can avoid resource waste.
