摘要
目的:探讨特发性膜性肾病(IMN)患者红细胞补体受体1(CR1)表达调节及其与临床病理之间关系。方法:选取2023年1月~2024年1月在广西壮族自治区南溪山医院肾内科住院确诊为IMN的患者44例作为研究组,另随机选取门诊无偿献血屋健康献血者41例作为对照组进行前瞻性研究,采用双抗体夹心酶联免疫吸附法分别测定两组红细胞膜唾液含量,同时收集病例组相关临床病理资料。结果:研究组患者的血红蛋白含量淋巴细胞计数、红细胞膜CR1含量明显低于对照组,差异均有统计学意义(P<0.05)。研究组内比较,红细胞膜CR1含量与24 h尿蛋白定量、血清肌酐、血清抗磷脂酶A2受体抗体(抗PLA2R抗体)呈负相关(r<0,P<0.05),与血红细胞计数、血白细胞计数、血红蛋白含量、血淋巴细胞计数、血中性粒细胞、血清IgG、血清白蛋白、肾脏免疫病理C3沉积等呈正相关(r>0,P<0.05);与肾脏病理分期、免疫病理IgG、IgG3、IgG4、IgM沉积阳性强度无相关性(P>0.05)。结论:IMN患者红细胞膜唾液酸表达下调,与临床关系较为紧密,对于临床具有重要价值。
Objective To explore change of erythrocytic membrane CR1 content and evaluate the change’s correlations with clinical and renal pathology in patiens with idiopathic membranous nephropathy.Methods Collecting 44 patients with idiopathic membranous nephropathy as a case group,and 41 healthy volunteers as a control group in the Nanxishan Hospital of Guangxi Zhuang Autonomous Region from January 2023 to January 2024.The erythrocytic membrane sialic acid content in the two groups was determined using an enzyme-linked immunosorbent assay.Results We found that the HB and lymphocyte counts of the case and control groups were(123.136±18.261)g/L and(133.095±9.048)g/L,(1.649±0.656)×10^(9)/L and(1.958±0.591)×10^(9)/L respectively.Compared to healthy volunteers in the control group,HB and lymphocyte count patients in the case group were significantly different(P<0.05),which showed HB and lymphocyte count patients in case group are lower;Erythrocytic membrane CR1 content of case and control groups are(3355.087±1196.446)pg/ml and(4055.472±1269.228)pg/ml respectively.Compared to healthy voluteers in control group,erythrocytic membrane CR1 content of patie ns in case group is significantly different(P<0.05),which showed lower;In case group,the amount of urinary protein a day,serum creatimine,anti-phospholipase A2 receptor antibody in the serum were negative correlations with erythrocytic membrane CR1 content;erythrocyte count,leukocyte count,lymphocyte count,neutrophil count and haemo-globin in blood routine examination,serum IgG,serum albuminand and the degree of C3 deposition in renal immunohistochemistry were positive correlations with erythrocytic membrane sialic acid content.The change of erythrocytic membrane CR1 didn’t have a significant correlation with the other renal pathology.Conclusions Modulation of erythrocytic membrane CR1 has a close correlation with clinical results and plays an essential role in idiopathic membranous nephropathy.However,it has an unclear correlation with renal pathology ex-cept for the degree of C3 deposition.
