摘要
目的分析益母生化汤联合地屈孕酮用于宫腔镜子宫内膜息肉切除术后患者的临床效果。方法回顾性分析2022年10月至2024年10月许昌中医院收治的宫腔镜子宫内膜息肉切除术后患者82例,按照随机数字表法分为对照组和观察组,各41例。对照组予以地屈孕酮,观察组在对照组基础上加用益母生化汤。对比两组疗效、中医证候积分、子宫指标、性激素指标、炎性因子[白细胞介素(IL)-4、IL-6、肿瘤坏死因子-α(TNF-α)]水平、不良反应、复发率。结果观察组治疗率为92.68%(38/41),高于对照组的73.17(30/41);用药后,观察组经色黯淡、经行腹痛、面色灰暗分别为(0.51±0.32)分、(0.46±0.29)分、(0.48±0.31)分,卵泡刺激素(FSH)、孕酮(P)、雌二醇(E2)、促黄体生成素(LH)水平分别为(4.54±0.28)mIU/mL、(0.32±0.05)ng/mL、(33.68±2.15)pg/mL、(4.27±0.45)mIU/mL,IL-6、TNF-α分别为(258.53±10.28)ng/L、(31.47±1.28)ng/L,均低于对照组的(1.72±0.45)分、(1.53±0.38)分、(1.85±0.43)分、(5.85±0.34)mIU/mL、(0.53±0.06)ng/mL、(47.72±2.64)pg/mL、(5.32±0.48)mIU/mL、(317.64±13.45)ng/L、(42.58±2.05)ng/L;经期为(4.74±0.56)d,短于对照组的(6.25±0.58)d;月经量为(58.05±4.24)mL,子宫内膜厚度为(6.17±0.46)mm,少于对照组的(67.45±5.06)mL、(8.25±0.53)mm;IL-4为(91.54±6.21)ng/L,高于对照组的(73.62±4.49)ng/L,有统计学差异(P<0.05);两组不良反应发生率对比无统计学差异(P>0.05);观察组复发率为4.88%(2/41),低于对照组的24.39%(10/41)(P<0.05)。结论益母生化汤联合地屈孕酮可提高宫腔镜子宫内膜息肉切除术后患者的治疗效果,减轻临床症状,改善子宫指标,调节性激素水平,减轻炎症症状,降低复发率。
Objective To analyze the effect of Yimu Shenghua decoction combined with dexmedetomidine on patients after hysteroscopic endometrial polypectomy.Methods Eighty-two patients after hysteroscopic endometrial polypectomy in Xuchang Traditional Chinese Medicine Hospital from October 2022 to October 2024 were retrospectively analyzed,and they were randomly divided into the control group and observation group according to the random number table method,with 41 cases in each group.The control group was treated with dydrogesterone,and the observation group was treated with Yimu Shenghua decoction on the basis of the control group.The two groups were compared in terms of therapeutic efficacy,traditional Chinese medicine evidence points,uterine indexes,sex hormone indexes,levels of inflammatory factors[interleukin(IL)-4,IL-6,tumor necrosis factor-α(TNF-α)],adverse reactions,and recurrence rate.Results The treatment rate of the observation group was 92.68%(38/41),which was higher than the that of the control group[73.17%(30/41)].After medication,the observation group had dull complexion,abdominal pain,and dark complexion with scores of(0.51±0.32)points,(0.46±0.29)points,and(0.48±0.31)points,respectively,the levels of follicle stimulating hormone(FSH),progesterone(P),estradiol(E2),and luteinizing hormone(LH)were(4.54±0.28)mIU/mL,(0.32±0.05)ng/mL,(33.68±2.15)pg/mL,and(4.27±0.45)mIU/mL,respectively;The levels of IL-6 and TNF-αwere(258.53±10.28)ng/L and(31.47±1.28)ng/L,respectively,which were lower than those of the control group[(1.72±0.45)points,(1.53±0.38)points,(1.85±0.43)points,(5.85±0.34)mIU/mL,(0.53±0.06)ng/mL,(47.72±2.64)pg/mL,(5.32±0.48)mIU/mL,(317.64±13.45)ng/L,(42.58±2.05)ng/L];Menstrual period was(4.74±0.56)d in observation group,which was shorter than that of the control group[(6.25±0.58)d];Menstrual flow was(58.05±4.24)mL,endometrial thickness was(6.17±0.46)mm in observation group,which were less than those of the control group[(67.45±5.06)mL,(8.25±0.53)mm];IL-4 was(91.54±6.21)ng/L in observation group,which was higher than that of the control group[(73.62±4.49)ng/L],with statistical difference(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05);The recurrence rate of the observation group was 4.88%(2/41),which was lower than the 24.39%(10/41)of the control group(P<0.05).Conclusion The combination of Yimu Shenghua decoction and dexmedetomidine can improve treatment efficacy,alleviate clinical symptoms,improve uterine indicators,regulate sex hormone levels,alleviate inflammatory symptoms,and reduce recurrence rate.
