摘要
目的分析双颌垫(TB)矫治器干预治疗小儿腺样体肥大口呼吸所致安氏Ⅱ类Ⅰ分类错颌畸形的临床效果。方法选取2021年10月—2023年6月我院收治的54例腺样体肥大口呼吸所致安氏Ⅱ类Ⅰ分类错颌畸形患儿,根据随机数字表法简单随机将其分为对照组(n=27,肌激动器治疗)和观察组(n=27,TB矫治器治疗)。比较两组患者临床治疗总有效率、口腔不良习惯改善情况和头影测量参数水平。结果与对照组比,观察组佩戴舒适性、佩戴顺利及佩戴依从性得分及口腔不良习惯改善总有效率均较高(P<0.05)。观察组AO-BO、ANB、Z-ANGLE均低于对照组和治疗前(P<0.05),观察组FMIA、FMA、OCC-PLNE、PFH、AFH均高于对照组和治疗前(P<0.05);两组间SNB水平比较差异无统计学意义(P>0.05)。结论对小儿腺样体肥大口呼吸所致的安氏Ⅱ类Ⅰ分类错颌畸形给予TB矫治器治疗可取得较好效果,在口腔不良习惯及上下颌骨、软组织方面均有较好的改善作用。
Objective To analyze the clinical effect of double jaw pad(TB)appliance in the treatment of Ambrosia ClassⅡⅠmalocclusion caused by oral respiration in children with adenoid hypertrophy.Methods All enrolled patients were 54 children with ClassⅡⅠmalocclusion caused by oral respiration of adenoid hypertrophy admitted to our hospital from October 2021 to June 2023.They were randomly divided into control group(n=27,myoagonist treatment)and observation group(n=27,TB treatment)according to random number table method.The total effective rate of clinical treatment,the improvement of oral bad habits and the level of cephalic shadow measurement parameters were compared between the two groups.Results Comparison of the treatment effect between the two groups:the scores of wearing comfort,wearing smoothly and wearing compliance in the observation group were higher than those in the control group(P<0.05).Comparison of the improvement of oral bad habits between the two groups:the total effective rate of the improvement of oral bad habits in the observation group was 88.89%,which was higher than that in the control group 66.17%(P<0.05).Comparison of cephalic image measurement parameters between the two groups:Before treatment,there was no statistical significance in the level of cephalic image measurement parameters between the two groups(P>0.05).After treatment,AO-BO,ANB and Z-ANGLE of both groups were decreased(P<0.05),and observation group was lower than control group(P<0.05),FMIA,FMA,OCC-PLNE,PFH and AFH of both groups were increased(P<0.05),and observation group was higher than control group(P<0.05).There was no significant difference in SNB level between the two groups(P>0.05).Conclusion TB orthodontic device is effective in the treatment of Ambrosia ClassⅡⅠmalocclusion caused by oral respiration of adenoid hypertrophy in children,and can improve the oral habits,mandible and soft tissue of children.
