摘要
目的:研究无托槽隐形矫治器治疗青少年下颌后缩型安氏Ⅱ类Ⅰ分类错颌的临床效果。方法:回顾性分析2020年3月—2022年12月平顶山学院附属口腔医院收治的80例下颌后缩型安氏Ⅱ类Ⅰ分类错颌患者的临床资料,按治疗方法的不同将其分为对照组和观察组,每组各40例。对照组患者行直丝弓固定器矫治治疗,观察组患者行无托槽隐形矫治器治疗。比较两组患者矫治时间、术前术后头影测量参数[下齿槽座角(SNB)、上齿槽座角(SNA)、上下齿槽座角(ANB)]水平、术前术后颞下颌关节参数水平、术前术后生活质量评分及并发症状况。结果:观察组患者患牙矫治时间短于对照组,差异有统计学意义(t=4.363,P<0.05)。术后1个月,观察组患者生活质量评分较对照组低,差异均有统计学意义(t=3.807、5.598、4.382、4.137,P<0.001)。结论:下颌后缩型安氏Ⅱ类Ⅰ分类错颌患者佩戴无托槽隐形矫治器治疗效果显著,能缩短矫治周期,改善颞下颌关节参数和头影测量参数,提高生活质量,安全性较好。
Objective:To study the clinical effect of invisible orthodontic appliances without brackets on the treatment of Class Ⅱ and Class I malocclusion in adolescents with mandibular retrusion.Methods:80 patients with Class Ⅱ and Class Ⅰ malocclusion with mandibular retrognathia admitted to the hospital from March 2020 to December 2022 were selected.They were divided into two groups based on different treatment methods,with 40 cases in each group.The control group received orthodontic treatment with straight wire arch fixators,while the observation group received treatment with invisible braces without brackets.The treatment time,preoperative and postoperative cephalometric parameters[lower alveolar seating angle(SNB),upper alveolar seating angle(SNA),upper and lower alveolar seating angle(ANB)]levels,preoperative and postoperative temporomandibular joint parameter levels,preoperative and postoperative quality of life scores,and complications between two groups were compared.Results:Compared with the control group,the observation group had a shorter mandibular retraction correction time,and the difference was statistically significant(t=4.363,P<0.05).1 month after surgery,compared with the control group,the observation group had lower scores on the health impact scale and better quality of life,with statistically significant differences(t=3.807,5.598,4.382,4.137;P<0.001).Conclusion:Patients with Class Ⅱ and Class Ⅰ malocclusion with mandibular retrusion who wear invisible orthodontic appliances without brackets have significant therapeutic effect.They can shorten the correction cycle,improve temporomandibular joint parameters and cephalometric measurement parameters,improve quality of life,and have good safety.
