摘要
目的本研究旨在探索快速康复理念应用于膝关节镜手术围手术期护理中的应用效果。方法2023年7月至2024年7月从烟台海港医院骨科住院部选取112例膝关节镜手术患者,采用随机数字表法均分为两组,即对照组(56例)和观察组(56例)。对照组患者采用常规护理,观察组则实施以快速康复理念为核心的围手术期干预,对比两组患者术中及术后不同时期疼痛评分,统计术后并发症发生率,记录首次下地活动时间及总住院时间,并于术前和术后第4周评估膝关节功能、膝关节活动度。结果两组患者术前及术后6 h视觉模拟评分(VAS)对比,差异无统计学意义(P>0.05),而在术后12 h、24 h、48 h VAS评分低于对照组(P<0.05)。观察组围手术期并发症发生率、首次下床活动时间、住院时间、术后4周Lysholm膝关节评分(LKS)评分、膝关节活动度(ROM评分)评分均优于对照组(P<0.05)。结论将快速康复理念引入膝关节镜手术患者的围手术期护理实践,有助于减轻术后疼痛程度、降低并发症发生率、缩短住院周期,并对术后膝关节功能改善及活动度提高具有积极促进作用。
Objective This study aims to explore the application effect of rapid rehabilitation concept in perioperative nursing of knee arthroscopic surgery.Methods From July 2023 to July 2024,112 patients undergoing knee arthroscopy were selected from the Orthopedic inpatient Department of Yantai Port Hospital.The patients were evenly divided into two groups by random number table method,namely the control group(56 cases)and the observation group(56 cases).Patients in the control group received routine nursing,while patients in the observation group received perioperative intervention with rapid rehabilitation concept as the core.The pain scores during operation and at different periods after operation were compared between the two groups,the incidence of postoperative complications was counted,the time of first ambulation and the total length of hospital stay were recorded,and the knee function and knee range of motion were evaluated before operation and at 4 weeks after operation.Results There was no significant difference in visual analogue scale(VAS)between the two groups before operation and 6 hours after operation(P>0.05).The VAS scores at 12 hours,24 hours and 48 hours after operation were lower than those of the control group(P<0.05).The incidence of perioperative complications,the first ambulation time,hospitalization time,and the Lysholm knee score(LKS)and knee range of motion(ROM)scores at 4 weeks after operation in the observation group was better than that in the control group(P<0.05).Conclusions The concept of enhanced recovery after surgery is introduced into the perioperative nursing practice of patients undergoing knee arthroscopic surgery,which helps to reduce the degree of postoperative pain,reduce the incidence of complications,shorten the hospitalization period,and has a positive effect on the improvement of knee joint function and activity.
