摘要
本研究目的是探讨桂枝茯苓丸联合醋酸亮丙瑞林对子宫内膜异位症患者中医证候、疼痛程度及T淋巴细胞亚群的影响。选取2021年1月—2022年9月医院收治的114例子宫内膜异位症患者分为亮丙瑞林组(57例,采用醋酸亮丙瑞林治疗)和桂枝茯苓丸组(57例,桂枝茯苓丸联合醋酸亮丙瑞林治疗),分组方法为随机抽签法,均治疗3个月,后随访12个月。统计两组治疗3个月后的临床疗效及随访期间复发情况,比较两组治疗前及治疗3个月后中医证候积分、疼痛程度、血清细胞因子水平、外周血T淋巴细胞亚群水平、子宫动脉血流动力学指标。治疗3个月后,桂枝茯苓丸组总有效率高于亮丙瑞林组(P<0.05);随访期间,桂枝茯苓丸组复发率低于亮丙瑞林组(P<0.05)。两组治疗3个月后头晕耳鸣、畏寒肢冷、月经不调、下腹胀痛、痛引腰骶、疼痛评分及血清单核细胞趋化蛋白1(MCP-1)、缺氧诱导因子-1α(HIF-1α)、转化生长因子β(TGF-β1)、碱性成纤维细胞生长因子(bFGF)、外周血CD8^(+)水平、子宫动脉阻力指数与治疗前相比,明显降低,桂枝茯苓丸组低于亮丙瑞林组(P<0.05)。两组治疗3个月后外周血CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)、子宫动脉血流量、平均流速与治疗前相比,明显升高,桂枝茯苓丸组高于亮丙瑞林组(P<0.05)。桂枝茯苓丸联合醋酸亮丙瑞林对子宫内膜异位症患者进行治疗,可有效降低患者中医证候,缓解疼痛,改善血清细胞因子水平,调节T淋巴细胞亚群水平,改善患者免疫功能,具有较好的治疗效果,且可有效降低复发的风险。
To study the effects of Guizhi Fuling Pill combined with leuprorelin acetate on traditional Chinese medicine symptomss,pain degree and T lymphocyte subsets in patients with endometriosis,from January 2021 to September 2022,114 patients with endometriosis in the hospital were divided into the leuprorelin group(57 cases were treated with leuprorelin acetate)and the Guizhi Fuling Pill group(57 cases were treated with Guizhi Fuling Pill combined with leuprorelin acetate).The grouping method was random lottery,and they were treated for 3 months and followed up for 12 months.The clinical efficacy and recurrence during the follow-up period of the two groups were counted,and the score of traditional Chinese medicine symptoms,the degree of pain,the level of serum cytokines,the level of T lymphocyte subsets in peripheral blood and the hemodynamic indexes of uterine artery were compared between the two groups before and after the treatment for 3 months.After 3 months of treatment,the total effective rate of the Guizhi Fuling Pill group was higher than that of the leuprorelin group(P<0.05).During the follow-up period,the recurrence rate of the Guizhi Fuling Pill group was lower than that of the leuprorelin group(P<0.05).After 3 months of treatment,scores of dizziness,tinnitus,chills of limbs,irregular menstruation,abdominal distension and pain,lumbosacral pain,pain,levels of serum monocyte chemoattractant protein-1(MCP-1),hypoxia inducible factor-1α(HIF-1α),transforming growth factor-β(TGF-β1),basic fibroblast growth factor(bFGF),CD8^(+)in peripheral blood in the two group decreased compared with before treatment,and that in the Guizhi Fuling Pill group was lower than that in the leuprorelin group(P<0.05).After 3 months'treatment,the levels of CD3^(+)and CD4^(+)in peripheral blood,CD4^(+)/CD8^(+),uterine artery blood flow and average flow rate in the two groups increased compared with before treatment,and that in the Guizhi Fuling Pill group was higher than that in the leuprorelin group(P<0.05).Guizhi Fuling Pill combined with leuprorelin acetate could effectively reduce patients'traditional Chinese medicine symptoms syndromes,relieve their pain,improve the level of serum cytokines,adjust the level of T lymphocyte subsets,and improve patients'immune function.It had a good therapeutic effect and could effectively reduce the risk of recurrence.
