摘要
探讨骨水泥诱导膜技术(induced membrane technique,IMT)对难愈性及继发立体型缺损创面患者住院及治疗时间的影响。选择2023—2025年玉林市第一人民医院收治的65例难愈性及继发立体型缺损创面患者为研究对象,并随机将其分为IMT组和VSD(vacuum sealing drainage,VSD)组,比较骨水泥诱导膜技术(IMT组,n=35)与传统清创+VSD治疗(VSD组,n=30)的临床疗效。结果显示,治疗后,IMT组创面闭合时间、总住院天数均短于VSD组,总治疗次数明显少于VSD组(P<0.05);治疗7天、14天,IMT组VAS评分明显低于VSD组(P<0.05);IMT组术后深部感染率、IL-6、CRP水平低于VSD组(P<0.05);IMT组各项直接成本均低于VSD组(P<0.05)。研究发现,骨水泥IMT产生的诱导膜能有效控制创面感染,稳定创面,利于创面早期修复,具有重要的临床应用价值。
This study aims to explore the impact of the bone cement-induced membrane technique(IMT)on the hospitalization and treatment duration of patients with refractory and secondary three-dimensional defect wounds.A total of 65 patients with refractory and secondary three-dimensional defect wounds admitted to The First People’s Hospital of Yulin from 2023 to 2025 were selected as research subjects,and they were randomly divided into the IMT group and the VSD group.The clinical efficacy of the bone cement-induced membrane technique(IMT group,n=35)and traditional debridement combined with VSD treatment(VSD group,n=30)was compared.The results showed that after treatment,the wound closure time and total hospitalization days in the IMT group were shorter than those in the VSD group,and the total number of treatments was significantly less than that in the VSD group(P<0.05).At 7 and 14 days after treatment,the VAS scores in the IMT group were significantly lower than those in the VSD group(P<0.05).The postoperative deep infection rate,IL-6,and CRP levels in the IMT group were lower than those in the VSD group(P<0.05).The direct costs such as material fees,operation fees,hospitalization fees,and total costs in the IMT group were all lower than those in the VSD group(P<0.05).The study concludes that the induced membrane generated by bone cement IMT can effectively control wound infection,stabilize the wound,facilitate early wound repair,significantly shorten the repair cycle of complex three-dimensional defects,thus having important clinical transformation value.
